胸腹腔镜联合食管癌切除并左侧颈部吻合术的效果及对病人营养水平的影响

    Effect of thoracic laparoscopy combined with resection of esophageal carcinoma and left cervical anastomosis and its effects on nutritional status of patients

    • 摘要: 目的:研究胸腹腔镜联合食管癌切除并左侧颈部吻合术效果及对病人营养水平影响。方法:将118例食管癌病人根据手术方式分为2组,各59例。对照组接受常规经左胸食管癌切除术联合胃食管胸内吻合术治疗,观察组实施胸腹腔镜联合食管癌切除并左侧颈部吻合术治疗。比较2组治疗效果。结果:术后1个月2组病人血清白蛋白、血红蛋白改善效果均优于术前(P<0.01),且观察组优于对照组(P<0.01);术后3个月,观察组生活质量较对照组更好(P<0.01),术后并发症总发生率(6.8%)低于对照组(22.0%)(P<0.05)。结论:胸腹腔镜联合食管癌切除并左侧颈部吻合术治疗食管癌效果显著,并发症少,能有效改善病人营养水平及生活质量,促进病人恢复。

       

      Abstract: Objective:To investigate the effects of thoracic laparoscopy combined with esophagectomy and left cervical anastomosis,and its effect on nutritional status of patients.Methods:One hundred and eighteen esophageal cancer patients were divided into the control group and observation group according to the operation method(59 cases each group).The control group were treated with conventional thoracic esophageal cancer resection combined with gastroesophageal anastomosis,and the observation group were treated with thoracic laparoscopy surgery combined with esophageal cancer resection and left neck anastomosis.The treatment effect between two groups were compared.Results:The improvement of serum ALB and Hb in two groups after 1 and 2 months of operation were better than those before operation(P<0.01),and which in observation group was better than that in control group(P<0.01).After 3 months of operation,the quality of life in observation group was significantly higher than that in control group(P<0.01).The total incidence of complications in observation group(6.8%) was lower than that in control group(22.0%)(P<0.05).Conclusions:Thoracoscopy combined with esophagectomy and left cervical anastomosis in the treatment of esophageal cancer is effective,and few complication,which can improve the nutritional level and quality of life,and promote the recovery of patients.
