    DoseRight技术条件下不同定位像对儿童腹部CT辐射剂量及图像质量的影响

    Effect of different positioning images on the radiation dose and image quality of abdominal CT in children under DoseRight technology

      目的比较采用正位定位像、侧位定位像和双定位像在DoseRight技术条件下进行儿童腹部CT检查时对图像质量和辐射剂量的影响。
      方法前瞻性收集行腹部CT检查的1~17岁患儿388例,依据患儿年龄分为1~6岁组(A组)、7~12岁组(B组)和13~17岁组(C组),每组患儿采用随机数字法分别分配到正位定位像组、侧位定位像组和双定位像组。采用飞利浦128 iCT进行数据采集,记录患儿的扫描范围和容积CT剂量指数(CTDIVOL),计算其体型特异性剂量估算值(SSDE),对CT图像质量进行主观及客观评价。
      结果3组患儿组内各小组间CTDIVOL和SSDE差异均有统计学意义(P < 0.01),均为正位定位像组高于侧位定位像组,侧位定位像组高于双定位像组(P < 0.05)。各组患儿图像质量主观评分均≥3分,满足诊断需求,3组患儿组内各小组间图像质量主观评分差异均无统计学意义(P>0.05)。
      结论进行儿童腹部CT检查时采用双定位像进行扫描能够更精确控制扫描范围同时降低辐射剂量并保证图像质量。

       

      Abstract:
      ObjectiveTo compare the effects of the anteroposteric, lateral and dual positioning images on the image quality and radiation dose in children abdominal CT examination under DoseRight technology.
      MethodsThe data of abdominal CT examination in 388 children aged 1-17 years were prospectively analyzed.According to the age, the children were divided into the 1-6 years old group(group A), 7-12 years old group(group B) and 13-17 years old group(group C).The children in each group were assigned to the anteroposteric, lateral and double positioning groups by random number method.The Philips 128 iCT was used for the data collection, the scan range and volume CT dose index(CTDIVOL) were recorded, the size-specific dose estimate(SSDE) were calculated, and the image qualities were subjectively and objectively evaluated in three groups.
      ResultsThe differences of CTDIVOL and SSDE among three groups were statistically significant(P < 0.01), which in anteroposterior positioning group were higher than those in lateral positioning group, and which in lateral positioning group were higher than those in dual positioning group(P < 0.05).The subjective scores of the image quality in three groups were more than or equal to 3 points, which met the diagnostic requirements.There was no statistical significance in the subjective score of image quality among three groups(P>0.05).
      ConclusionsThe application of dual positioning images in children abdominal CT examination can accurately control the scan range, reduce the radiation dose and ensure the image quality.
