    序贯肠内营养联合益生菌在重型颅脑损伤病人中的应用效果

    Application effect of sequential enteral nutrition combined with probiotics in patients with severe head injury

      目的探讨序贯肠内营养联合益生菌在重型颅脑损伤病人中的应用效果。
      方法选取重型颅脑损伤病人120例,按照系统抽样法随机分为对照组和观察组,各60例。对照组病人给予序贯肠内营养支持治疗,观察组在对照组基础上联合益生菌治疗。干预治疗2周后,比较2组病人的营养状况、胃肠功能情况、神经功能恢复效果以及胃肠道并发症发生情况。
      结果干预治疗后,观察组病人血清血清总血红蛋白、白蛋白、转铁蛋白、前白蛋白水平均明显高于对照组(P <0.01),序贯器官衰竭量表评分、急性生理学及慢性健康状况量表评分均明显低于对照组,格拉斯哥昏迷评分明显高于对照组(P <0.01);观察组肠内营养达标时间、首次排便时间和住院时间均明显短于对照组(P <0.01);观察组胃肠道并发症发生率、肺部感染发生率、泌尿系统感染率及胃潴留量均低于对照组(P <0.05~P <0.01)。
      结论采用序贯肠内营养支持联合益生菌对重症颅脑损伤病人进行治疗,有助于改善病人营养状态,提高胃肠动力水平,减少胃肠道并发症发生,值得临床推广应用。

       

      Abstract:
      ObjectiveTo explore the effect of sequential enteral nutrition combined with probiotics in the treatment of severe craniocerebral injury.
      MethodsTotal 120 patients with severe craniocerebral injury admitted to our hospital were randomly divided into control group(60 cases) and observation group(60 cases) according to the systematic sampling method.The patients in the control group were treated with sequential enteral nutrition support, while the patients in the observation group were treated with probiotics on the basis of sequential enteral nutrition support.After 2 weeks of intervention, the nutritional status, gastrointestinal function, neurological recovery and gastrointestinal complications of the two groups were compared.
      ResultsAfter intervention, the levels of serum total hemoglobin, albumin, transferrin and prealbumin in the observation group were significantly higher than those in the control group(P <0.01).The scores of sequential organ failure assessment, acute physiology and chronic health evaluation were significantly lower than those in the control group, and the scores of Glasgow Coma Score were significantly higher than that in the control group(P <0.01).The standard time of enteral nutrition, the first defecation time and the hospital stay in the observation group were significantly shorter than those in the control group(P <0.01).Hair coloring rate, pulmonary infection rate, urinary tract infection rate and gastric retention rate in observation group were lower than those in control group(P <0.05 to P <0.01).
      ConclusionsUsing sequential enteral nutrition support combined with probiotics to treat severe craniocerebral injury patients can significantly improve the nutritional status of patients, improve gastrointestinal function, improve the level of gastrointestinal motility, reduce the occurrence of gastrointestinal complications, improve the treatment effect and promote the prognosis, which is worthy of clinical application.
