新生儿重症监护病房患儿中等长度导管相关并发症影响因素分析

    Analysis of influencing factors of complications related to medline catheter in NICU children

      目的 探讨新生儿重症监护病房(NICU)患儿使用中等长度导管的并发症发生情况及其影响因素。
      方法 选取2021年2月至2022年6月某三甲医院NICU收治的178例应用中等长度导管的新生儿作为研究对象, 根据是否发生导管相关并发症将患儿分为发生组和未发生组, 收集2组患儿的基本信息、置管情况、固定方式等资料, 采用单因素分析与二元多因素logistic回归因素分析的方法, 分析新生儿中等长度导管并发症发生的相关因素。
      结果 178例中发生组33例, 并发症的发生率18.53%, 单因素分析结果显示: 出生体质量、穿刺静脉、导管尖端位置、导管留置时间及内固定与新生儿中等长度导管并发症的发生有关(P < 0.05~P < 0.01)。多因素回归分析结果显示: 选择颞浅静脉和头静脉、导管尖端位置、固定方式是新生儿中等长度导管相关并发症的独立危险因素(P < 0.05)。
      结论 新生儿进行中等长度导管应用时, 选择贵要静脉或肘正中静脉进行穿刺, 导管尖端位置在锁骨下静脉或腋静脉胸壁段, 固定方式选择内固定, 可降低新生儿中等长度导管并发症的发生率。

       

      Abstract:
      Objective To investigate the incidence and influencing factors of complications of midline catheter in children in neonatal intensive care unit (NICU).
      Methods A total of 178 newborns with midline catheter admitted to NICU of a tertiary hospital from February 2021 to June 2022 were selected as the study subjects.According to whether catheter-related complications occurred, the children were divided into the occurrence group and the non-occurrence group, and the basic information, catheterization conditions, fixation methods and other data of the two groups were collected.Univariate analysis and binary logistic regression analysis were used to analyze the related factors of neonatal midline catheter complications.
      Results Among the 178 cases, 33 cases were in the occurrence group, and the incidence of complications was 18.53%.Univariate analysis showed that birth weight, puncture vein, catheter tip location, catheter retention time and internal fixation were related to the occurrence of neonatal midline catheter complications (P < 0.05 to P < 0.01).Multivariate regression analysis showed that the selection of superficial temporal vein and cephalic vein for puncture, catheter tip location in jugular vein, and no internal fixation were independent risk factors for neonatal complications related to midline catheter (P < 0.05).
      Conclusions When the midline catheter is used in neonates, the insertion of the noble vein or the median cubital vein, the distal position of the catheter tip in the subclavian vein or the axillary vein, and the fixation method of internal fixation can reduce the incidence of complications of the midline catheter in neonates.
