Abstract:
Objective: To investigate the hemostatic efficacy of bilateral uterine artery suturing combined with balloon fillling for postpartum hemorrhage after cesarean section with placenta previa.
Methods: The medical records of 98 patients with hemorrhage after caesarean section with placenta previa were retrospectively analyzed,and the patients were divided into three groups according to the treatment methods.The group A(34 cases) was treated with balloon tamponade,the group B(38 cases) was treated with ascending branch of uterine artery ligation combined with balloon tamponade,and the group C(36 cases) was treated with bilateral uterine artery suture combined with balloon tamponade.The hemostatic related indexes,hemostatic effect,complications,coagulation indexesprothrombin time(PT) and fibrinogen(FIB),sex hormonesfollicle-stimulating hormone (FSH) and estradiol (E2) among three groups were compared.
Results: The operation time of group A was shorter than that of group B and group C(
P<0.05),and the amount of intraoperative blood loss,24 h postoperative blood loss and blood transfusion in groups A were higher than those in group B and group C(
P<0.05).The amount of intraoperative blood loss,24 h postoperative blood loss and blood transfusion in group B were higher than those in group C(
P<0.05).Twenty-six cases with effective hemostasis in group A were found,and the effective rate was 76.47%.Thirty-four cases with effective hemostasis in group B were found,and the effective rate was 89.47%.Thirty-five cases with effective hemostasis in group C were found,and the effective rate was 97.22%.The differences of the effective hemostasis among three groups were statistically significant(
P<0.05),and which in group A was lower than that in group B and group C(
P<0.05).The proportion of postoperative fever,poor uterine involution and diffuse intravascular coagulation in group A was higher than those in group B and group C(
P<0.05).After operation,the PT and FIB in three groups decreased,and those in group B and group C were lower than those in group A(
P<0.05).After operation,the FSH increased,and the E2 decreased in three groups(
P<0.05),and there was no statistical significance in the FSH and E2 among three groups(
P>0.05).
Conclusions: The hemostatic effect of bilateral uterine artery suture or ascending branch ligation of uterine artery combined with balloon tamponade in the treatment of postpartum hemorrhage after cesarean section with placenta previa is good.
