血清miR-206对铜绿假单胞菌感染肺炎诊断的临床意义

    Clinical significance of serum miR-206 in the diagnosis of pneumonia caused by Pseudomonas aeruginosa infection

    • 摘要: 目的:通过检测铜绿假单胞菌(PA)肺炎和健康者的血清miRNA表达谱,探寻血清中特异性表达的miRNA-206在区分细菌感染、感染严重程度的临床应用意义。方法:本研究招募了95例PA感染肺炎病人和90名健康人。通过定量聚合酶链反应PCR确认2组间差异表达的miRNA。通过受试者工作曲线以及logistic回归分析评估差异表达的血清miRNA区分PA感染病人和健康者以及不同程度PA感染的病人价值。结果:与健康者相比,miR-206在PA感染肺炎病人血清中的相对表达显著升高(P<0.01)。ROC曲线下面积为0.768 (95%CI:0.699~0.863,P<0.01),最佳截断值为0.51时,miR-206对PA诊断的敏感性和特异性分别为57.70%和88.50%;logistic回归分析表明,miR-206水平可预测PA感染的严重程度(轻度与重度OR=0.545,95%CI:0.362~0.823,P<0.01);中度与重度OR=0.774,95%CI:0.639~0.936,P<0.01)。结论:血清miR-206可作为PA感染肺炎的鉴定、诊断和严重程度的评估的血清学指标。

       

      Abstract: Objective: To explore the clinical significance of serum miRNA-206 expression in differentiating bacterial infections and severity of infections by detecting serum miRNA expression profiles in Pseudomonas aeruginosa(PA) pneumonia and healthy individuals. Methods: Ninety-five patients with PA pneumonia infection and 90 healthy individuals were recruited for this study.The differentially expressed miRNAs between the two groups were confirmed by quantitative PCR.Receiver operating characteristic(ROC) curve and logistic regression analyses were performed to assess the ability of altered serum miRNAs to differentiate between patients with PA and healthy individuals as well as patients with different levels of infection. Results: Relative expressions of miR-206 were significantly higher in serum of patients with pneumonia than those in healthy individuals.Area under the ROC curve was 0.768 (95%CI:0.699-0.863,P<0.01).When the optimal cut-off value was 0.51,the sensitivity and specificity of miR-206 for the diagnosis of PA were 57.70% and 88.50%,respectively.Logistic regression analysis showed that miR-206 levels predicted the severity of PA infections (mild vs severe OR=0.545,95%CI:0.362-0.823,P<0.01;moderate vs severe OR=0.774,95%CI:0.639-0.936,P<0.01). Conclusions: Serum miR-206 can be used as a serological marker for identification,diagnosis and assessment of severity of PA induced pneumonia.
