Abstract:
Objective: To explore the application of information case management based on 4C theory in the follow-up of elderly patients with coronary heart disease after percutaneous coronary intervention (PCI).
Methods: Eighty-seven elderly patients with coronary heart disease were selected as the study subjects,and randomly divided into a control group (
n=45) and an observation group (
n=42).The control group received routine discharge follow-up,while the observation group received information case management based on 4C theory on the basis of routine discharge follow-up.The blood glucose,blood lipid,cardiac function indicators frailty score,and Seattle angina questionnaire of patients between the two groups before and after intervention were compared.
Results: After 6 months of intervention,the 6-minute walking distance,left ventricular ejection fraction,high-density lipoprotein C,and scores of all dimensions of Seattle angina questionnaire of patients in the observation group were higher than those in the control group (
P<0.05 to
P<0.01),while fasting blood glucose,total cholesterol,triglyceride,low-density lipoprotein C,and frailty score were lower than those in the control group (
P<0.05 to
P<0.01).
Conclusions: The information case management based on 4C theory can effectively improve the blood glucose and lipid levels of elderly patients with coronary heart disease after PCI,enhance the level of cardiac function,improve the degree of frailty and quality of life of patients,and promote the rehabilitation of patients,which is worthy of clinical promotion.
