相位角与胃、结直肠恶性肿瘤病人营养状况的关系

    Relationship between phase angle and nutritional status of patients with gastric and colorectal cancer

      目的 探讨相位角(PA)与胃、结直肠恶性肿瘤病人围手术期营养状况相关性。
      方法 选取89例胃、结直肠恶性肿瘤病人,通过营养风险筛查2002(NRS 2002)、病人主观整体评估量表(PG-SGA)进行入院时营养筛查与评估,完善体格测量、体成分检测和相关血液生化指标并进行比较。
      结果 89例病人低PA组37例,正常PA组52例,与正常PA组相比,低PA组体质量指数、上臂围、握力、血红蛋白、红细胞、淋巴细胞、身体总水分、蛋白质、无机盐、去脂体质量、细胞内水分、骨骼肌、基础代谢、身体细胞量、骨矿质、骨骼肌质量指数(SMI)较低(P < 0.05~P < 0.01)。PA与血红蛋白、红细胞计数、握力、细胞内水分、骨骼肌、SMI呈正相关关系,PA与NRS2002评分、PG-SGA评分呈负相关关系(P < 0.05~P < 0.01)。多元线性回归分析显示,握力、细胞内水分、SMI越大,骨骼肌含量越小,PA得分越大(P < 0.05~P < 0.01)。
      结论 PA能帮助识别早期存在营养风险的病人,病人可以早期得到干预,从而改善胃肠道恶性肿瘤病人的预后。

       

      Abstract:
      Objective To investigate the correlation between phase angle (PA) and perioperative nutritional status of patients with gastric and colorectal malignancies.
      Methods A total of 89 patients with gastric and colorectal malignant tumors were selected for nutritional screening and evaluation at admission through Nutrition Risk Screening 2002 (NRS 2002) and patient generated subjective global assessment (PG-SGA), so as to improve physical measurement, body composition detection and related blood biochemical indicators.
      Results There were 37 cases in the low PA group and 52 cases in the normal PA group in 89 patients with malignant tumors.Compared with the normal PA group, the body mass index, upper arm circumference, grip strength, hemoglobin, red blood cells, lymphocytes, total body water, protein, inorganic salts, fat free weight, intracellular water, skeletal muscle, basic metabolism, body cell mass, bone mineral, skeletal muscle mass index (SMI) in the low PA group were lower(P < 0.05 to P < 0.01).PA was positively correlated with hemoglobin, erythrocyte count, grip strength, intracellular moisture, skeletal muscle and SMI, while PA was negatively correlated with NRS2002 score and PG-SGA score (P < 0.05 to P < 0.01).
      Conclusions PA can help identify patients with early nutritional risk, and patients can get early intervention, so as to improve the prognosis of patients with gastrointestinal cancer.
