先兆子痫孕妇血清sVCAM-1、IFI16与胎盘血流及胎儿生长受限的相关性分析

    Correlation analysis of serum sVCAM-1, IFI16, placental blood flow and fetal growth restriction in pregnant women with preeclampsia

      目的 分析先兆子痫(PE)孕妇血清可溶性血管内皮细胞黏附分子-1(sVCAM-1)、干扰素诱导蛋白16(IFI16)表达情况与胎盘血流变化以及胎儿生长受限(FGR)的关系。
      方法 选取PE合并FGR孕妇76例作为观察组,同时收集同期PE孕妇80例作为对照A组,正常分娩孕妇80例作为对照B组。采用酶联免疫吸附(ELISA)法测定血清sVCAM-1、IFI16水平。Pearson相关性分析sVCAM-1、IFI16及二者与胎盘血流参数:搏动指数(PI)、阻力指数(RI)和收缩期峰值流速/舒张末期流速(S/D)的相关性。多因素logistic分析FGR发生的影响因素。
      结果 对照B组、对照A组、观察组血清sVCAM-1、IFI16、S/D、RI、PI水平依次显著升高(P<0.01)。PE孕妇血清sVCAM-1与胎盘血流参数S/D、RI、PI呈正相关(P<0.01),血清IFI16与胎盘血流参数S/D、RI、PI呈正相关(P<0.01),且血清sVCAM-1、IFI16水平呈正相关(P<0.01)。多因素logistic分析显示血清sVCAM-1、IFI16为FGR发生的危险因素(P<0.01)。
      结论 血清sVCAM-1、IFI16在PE孕妇中表达水平上调,二者与胎盘血流参数密切相关,且二者是FGR发生的危险因素。

       

      Abstract:
      Objective To analyze the expressions of serum soluble vascular cell adhesion molecule-1 (sVCAM-1) and interferon-inducible protein 16 (IFI16) in pregnant women with preeclampsia (PE) and their relationship to placental blood flow changes and fetal growth restriction (FGR).
      Methods Seventy-six pregnant women with PE combined with FGR were selected as the observation group.Eighty pregnant women with PE who underwent normal delivery during the same period were selected as the control group A. Eighty pregnant women who underwent normal delivery selected as the control group B.Serum sVCAM-1 and IFI16 levels were determined by enzyme-linked immunosorbent assay.Pearson correlation was applied to analyze the sVCAM-1, IFI16 and their correlation with placental blood flow parameters including pulsatility index (PI), resistance index (RI), and peak systolic/end-diastolic (S/D) flow.Multivariate logistic was applied to analyze the influencing factors of occurrence of FGR.
      Results The serum levels of sVCAM-1, IFI16, S/D, RI and PI in control group B, control group A and observation group were obviously increased in turn (P < 0.01).Pearson correlation analysis showed that serum sVCAM-1 in PE patients was positively correlated to placental blood flow parameters S/D, RI and PI (P < 0.01), serum IFI16 was positively correlated with placental blood flow parameters S/D, RI, PI (P < 0.01), the serum levels of sVCAM-1 and IFI16 in PE patients were positively correlated (P < 0.01).Multivariate logistic analysis showed that serum sVCAM-1 and IFI16 were risk factors for FGR (P < 0.01).
      Conclusions The expression levels of serum sVCAM-1 and IFI16 are up-regulated in PE patients, which are closely related to placental blood flow parameters, and both are risk factors for the occurrence of FGR.
