类风湿关节炎病人血清miR-210、LncRNA KCNQ1OT1表达及其临床诊断价值分析

    Expression and clinical diagnostic value of serum miR-210 and LncRNA KCNQ1OT1 in patients with rheumatoid arthritis

    • 摘要: 目的: 探讨类风湿关节炎(RA)病人血清微小RNA-210(miR-210)、长链非编码RNA KCNQ1重叠转录物1(LncRNA KCNQ1OT1)表达水平及其临床诊断价值。方法: 选择2020年8月至2021年12月收治的RA病人67例作为观察对象(RA组),其中低度活动组21例,中度活动组18例,高度活动组28例,另选取同期健康体检者60名作为对照组,qRT-PCR法检测血清miR-210、LncRNA KCNQ1OT1表达水平,收集病人一般资料及血清指标水平,Pearson法分析RA病人血清miR-210、LncRNA KCNQ1OT1与临床指标的相关性及miR-210与LncRNA KCNQ1OT1的相关性;ROC曲线分析miR-210、LncRNA KCNQ1OT1对RA的诊断价值。结果: 与对照组比较,RA组病人血清miR-210、RF、抗CCP抗体、CRP、ESR表达水平明显升高(P<0.01),LncRNA KCNQ1OT1表达水平明显降低(P<0.01);与低度活动组、中度活动组比较,高度活动组病人血清miR-210表达水平升高(P<0.01和P<0.05),LncRNA KCNQ1OT1表达水平降低(P<0.01和P<0.05),低度活动组和中度活动组病人血清miR-210、LncRNA KCNQ1OT1表达水平比较差异无统计学意义(P>0.05);血清miR-210表达水平与RA病人血清RF、CRP、抗CCP抗体、ESR水平及DAS28评分均呈正相关关系(P<0.01),LncRNA KCNQ1OT1表达水平与RA病人血清RF、CRP、抗CCP抗体、ESR水平及DAS28评分均呈负相关关系(P<0.01);RA病人血清miR-210与LncRNA KCNQ1OT1表达水平呈负相关关系(r=-0.532,P<0.05);miR-210、LncRNA KCNQ1OT1联合诊断RA的曲线下面积为0.949,敏感度为83.58%,特异度为93.33%。结论: RA病人血清miR-210呈高表达、LncRNA KCNQ1OT1低表达,且与RA疾病活动程度有关,有可能成为诊断RA的标志物。

       

      Abstract: Objective: To investigate the serum miR-210 and long non-coding RNA KCNQ1 overlapping transcript 1 (LncRNA KCNQ1OT)1 expression levels and their clinical diagnostic value in patients with rheumatoid arthritis (RA). Methods: Sixty-seven RA patients from August 2020 to December 2021 were selected as observation subjects (RA group),including 21 cases in the low activity group,18 cases in the moderate activity group,and 28 cases in the high activity group.In addition,60 healthy examinees were regarded as the control group.The expression levels of serum miR-210 and LncRNA KCNQ1OT1 were measured by qRT-PCR.The general information and serum index levels of the patients were collected.Pearson method was performed to analyze the correlation between serum miR-210,LncRNA KCNQ1OT1 and clinical indicators and the correlation between miR-210 and LncRNA KCNQ1OT1 in RA patients.ROC curve was performed to analyze the diagnostic value of miR-210 and LncRNA KCNQ1OT1 in RA. Results: Compared with the control group,the serum expression levels of miR-210,RF,anti-CCP antibody,CRP and ESR in the RA group were greatly raised(P<0.01),and the expression level of LncRNA KCNQ1OT1 was reduced (P<0.01).Compared with the low activity group and the moderate activity group,the expression level of serum miR-210 in the high activity group was greatly raised(P<0.01 and P<0.05),and the expression level of LncRNA KCNQ1OT1 was greatly reduced (P<0.01 and P<0.05).There was no significant difference in the expression levels of serum miR-210 and LncRNA KCNQ1OT1 between the low activity group and the moderate activity group (P>0.05).Serum miR-210 expression level was positively correlated with serum RF,CRP,anti-CCP antibody,ESR levels and DAS28 score in RA patients (P<0.01),LncRNA KCNQ1OT1 expression level was negatively correlated with serum RF,CRP,anti-CCP antibody,ESR levels and DAS28 score (P<0.01).Serum miR-210 in RA patients was negatively correlated with the expression level of LncRNA KCNQ1OT1 (r=-0.532,P<0.05).The area under the curve of miR-210 and LncRNA KCNQ1OT1 in combination to diagnose RA was 0.949,the sensitivity was 83.58%,and the specificity was 93.33%. Conclusions: Serum miR-210 in RA patients is highly expressed and LncRNA KCNQ1OT1 is lowly expressed,they are related to the degree of RA disease activity,and may be markers for the diagnosis of RA.
