高压氧辅助治疗三叉神经痛对病人血清SP、PGE2及炎症因子水平的影响

    Effect of hyperbaric oxygen on the serum levels of SP, PGE2 and inflammatory factors in patients with trigeminal neuralgia

    • 摘要: 目的: 分析高压氧辅助治疗三叉神经痛对病人血清P物质(SP)、前列腺素E2(PGE2)及炎症因子水平的影响。方法: 选取90例三叉神经痛病人为研究对象,随机数字表法分为观察组和对照组,各45例,对照组口服卡马西平配合封闭治疗,观察组在对照组治疗基础上加用高压氧辅助治疗。比较2组治疗效果比较、治疗前后血清SP、PGE2水平、炎症因子白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平、疼痛视觉模拟量表(VAS)、欧洲五维健康量表(EQ-5D)评分以及不良反应发生情况。结果: 观察组总有效率93.33%,高于对照组的77.78%(P < 0.05);治疗后,2组SP、PGE2水平较治疗前均下降(P < 0.05),且观察组SP、PGE2水平均低于对照组(P < 0.01);治疗后2组IL-1β、IL-6、TNF-α水平较治疗前均下降(P < 0.05),且观察组IL-1β、IL-6、TNF-α水平均低于对照组(P < 0.01);治疗后2组VAS评分较治疗前下降(P < 0.05),EQ-5D评分较治疗前上升(P < 0.05),观察组VAS评分低于对照组(P < 0.05),EQ-5D评分高于对照组(P < 0.05);2组不良反应发生率差异无统计学意义(P>0.05)。结论: 在常规治疗基础上联合高压氧进行辅助治疗可提高对三叉神经痛病人的治疗效果,进一步调节机体免疫及炎症反应,降低疼痛程度,改善生活质量,且无明显不良反应,值得临床推广。

       

      Abstract: Objective: To analyze the effects of hyperbaric oxygen on the serum levels of substance P(SP), prostaglandin E2(PGE2) and inflammatory factors in patients with trigeminal neuralgia. Methods: A total of 90 patients with trigeminal neuralgia were selected as the research subjects, and randomly divided into the observation group and control group(45 cases in each group).The control group was treated with carbamazepine by oral administration combined with block therapy, and the observation group were treated with hyperbaric oxygen on the basis of the control group.The treatment effect between two groups was compared, and the serum levels of SP, PGE2 and inflammatory factorinterleukin-1β(IL-1 β), interleukin 6(IL-6) and tumor necrosis factor-α(TNF-α) before and after treatment, scores of pain visual analog scale(VAS), European five-dimensional health scale(EQ-5D) and adverse reactions were compared between two groups. Results: The total effective rate in the observation group was 93.33%, which was significantly higher than that of control group(77.78%)(P < 0.05).After treatment, the levels of SP and PGE2 in two groups decreased compared with before treatment(P < 0.05), and the levels of SP and PGE2 in the observation group were lower than those in control group(P < 0.01).After treatment, the levels of IL-1β, IL-6 and TNF-α in two groups decreased compared with those before treatment(P < 0.05), and the levels of IL-1β, IL-6 and TNF-α in the observation group were lower than those in control group(P < 0.01).After treatment, the VAS scores in two groups were lower than that before treatment(P < 0.05), and the EQ-5D scores in two groups were higher than that before treatment(P < 0.05).After treatment, the VAS score in the observation group was lower than that in control group(P < 0.05), and the EQ-5D score was higher than that of in control group(P < 0.05).There was no statistical significance in the incidence rates of adverse reactions between two groups(P>0.05). Conclusions: Combined with hyperbaric oxygen on the basis of conventional treatment can significantly improve the therapeutic effect of patients with trigeminal neuralgia, further regulate the body′s immune and inflammatory response, reduce the degree of pain, improve the quality of life, and there is no obvious adverse reactions, it is worthy of clinical promotion.
