Abstract:
                                      Objective: To explore the application effect of health education based on knowledge-attitude-practice model combined with multidimensional collaborative continuity intervention in end stage renal disease (ESRD) patients undergoing hemodialysis. 
Methods: Eighty patients with ESRD undergoing hemodialysis were selected and randomly divided into a control group and an observation group,with 40 patients in each group.The control group received health education based on knowledge-attitude-practice model,while the observation group received multidimensional collaborative continuity intervention on the basis of the control group.Both groups were intervened for 2 months.The self-management ability,healthy behavior,hemodialysis complications,and quality of life were compared between the two groups before and after intervention. 
Results: After intervention,the self-management ability,healthy behavior,quality of life in the two groups were obviously improved compared with before intervention (
P<0.01),and the above indicators,incidence of hemodialysis complications in the observation group were significantly better than those in the control group (
P<0.05 to 
P<0.01). 
Conclusions: The combination of health education based on knowledge-attitude-practice model and multidimensional collaborative continuity intervention can enhance the nursing outcomes for patients with ESRD undergoing hemodialysis.This approach positively impacts patients' self-management ability and compliance,helps reducing complications,and improves patients' quality of life and satisfaction with nursing care.
