血清YKL-40、KLF5、CTGF联合检测对子宫内膜样腺癌病人的诊断价值

    Diagnostic value of combined detection of serum YKL-40, KLF5 and CTGF in patients with endometrial endometrioid carcinoma

      目的 探讨人血清几丁质酶-3样蛋白1(YKL-40)、Krüppel样因子5(KLF5)、结缔组织生长因子(CTGF)联合检测对子宫内膜样腺癌(EEC)的诊断价值。
      方法 选取2020—2021年88例经确诊的EEC病人为研究对象(EEC组),另选取84名正常体检人群为对照组。酶联免疫吸附(ELISA)法检测EEC病人血清中YKL-40、KLF5、CTGF水平。免疫组织化学法检测EEC病人癌组织和癌旁组织中YKL-40、KLF5、CTGF水平。采用多因素logistic回归分析影响EEC发生的危险因素。采用ROC曲线分析血清中YKL-40、KLF5、CTGF水平对EEC的诊断价值。
      结果 与对照组比较,EEC组血清中YKL-40、KLF5、CTGF表达明显升高(P < 0.01)。YKL-40、KLF5、CTGF联合诊断的ROC曲线下面积(AUC)明显大于YKL-40、KLF5、CTGF单独诊断的AUC(Z=3.448、3.885、3.101,P < 0.01)。与癌旁组织相比,EEC癌组织中YKL-40、KLF5、CTGF阳性表达明显升高(P < 0.01)。YKL-40、KLF5、CTGF表达与EEC分化程度、FIGO分期、淋巴结转移有关(P < 0.05~P < 0.01)。多因素logistic回归分析显示,YKL-40、KLF5、CTGF高水平是影响EEC发生的危险因素(P < 0.05~P < 0.01)。
      结论 YKL-40、KLF5、CTGF在EEC病人血清中表达水平均升高,三者是影响EEC发生的危险因素,三者联合对于EEC具有较高的诊断价值。

       

      Abstract:
      Objective To investigate the diagnostic value of combined detection of human serum chitinase-3-like protein 1(YKL-40), Krüppel-like factor 5(KLF5) and connective tissue growth factor(CTGF) in the endometrial endometrioid carcinoma(EEC).
      Methods Eighty-eight patients with EEC A total of from 2020 to 2021, and 84 patients with normal physical examination were divided into the EEC group and control group. The enzyme-linked immunosorbent assay was used to detect the levels of serum YKL-40, KLF5, and CTGF in EEC patients. The immunohistochemistry was used to detect the levels of YKL-40, KLF5, and CTGF in EEC patients' cancer tissues and adjacent tissues. Multivariate logistic regression was used to analyze the risk factors for EEC occurrence. ROC curve was used to analyze the diagnostic value of serum YKL-40, KLF5, and CTGF levels for EEC.
      Results Compared with the control group, the expression levels of serum YKL-40, KLF5 and CTGF in the EEC group obviously increased(P < 0.01). The area under the ROC curve(AUC) of combined diagnosis of YKL-40, KLF5, and CTGF was obviously larger than that of YKL-40(Z=3.448, P < 0.01), KLF5(Z=3.885, P < 0.01) and CTGF(Z=3.101, P < 0.01). Compared with adjacent tissues, the positive expressions of YKL-40, KLF5 and CTGF in EEC cancer tissues obviously increased(P < 0.05 to P < 0.01). The expressions of YKL-40, KLF5, and CTGF were related to the degree of EEC differentiation, FIGO stage and lymph node metastasis(P < 0.05 to P < 0.01). The results of multivariate logistic regression analysis showed that the high levels of YKL-40, KLF5 and CTGF were the risk factors of EEC(P < 0.05 to P < 0.01).
      Conclusions The expression levels of serum YKL-40, KLF5 and CTGF in EEC patients elevate, and the three are the risk factors of affecting the occurrence of EEC. The combination of the three has a high diagnostic value for EEC.
