血清STIM1联合子宫动脉血流动力学参数对妊娠期高血压疾病的诊断价值

    Diagnostic value of serum STIM1 combined with uterine arterial hemodynamics parameters in hypertensive diseases during pregnancy

      目的 探讨血清中基质相互作用分子1(STIM1)的表达水平,并分析其联合子宫动脉血流动力学参数对妊娠期高血压疾病的诊断价值。
      方法 选取接受治疗的112例妊娠期高血压孕妇作为观察组,根据病情严重程度分为妊娠期高血压组48例、子痫前期组34例和重度子痫前期组30例,同时间选择产检正常妊娠孕妇84例纳入对照组。采用酶联免疫吸附法检测各研究对象血清STIM1的表达水平;采用超声诊断仪检测子宫动脉血流阻力指数(RI)、搏动指数(PI)、收缩期与舒张期流速比值(S/D);血清STIM1水平与子宫动脉血流参数的关系通过Pearson相关性进行分析;ROC曲线分析血清STIM1联合子宫动脉血流动力学参数对重度子痫前期的诊断价值。
      结果 与对照组相比,观察组血清STIM1水平、RI、PI、S/D值均升高(P < 0.05)。且随着病情严重程度的增加,血清STIM1水平、RI、PI、S/D值均升高(P < 0.05)。Pearson相关性分析显示,血清STIM1水平与RI、PI、S/D值均呈正相关关系(r=0.515、0.620、0.592,P < 0.05)。ROC曲线结果显示,血清STIM1联合子宫动脉血流动力学参数诊断重度子痫前期的曲线下面积为0.951,高于RI、PI、S/D及血清STIM1单独检测(P < 0.05)。
      结论 妊娠期高血压疾病孕妇子宫动脉血流动力学参数RI、PI、S/D升高,血清中STIM1也升高,子宫动脉血流动力学参数联合血清STIM1检测对重度子痫前期具有更高的诊断价值。

       

      Abstract:
      Objective To investigate the expression levels of serum stromal interaction molecule 1(STIM1) and to evaluate the diognostic value of its combination with uterine artery hemodynamic parameters for hypertensive disorders of pregnancy.
      Methods One hundred and twelve arterial hemodynamics pregnant women with hypertensive disorders of pregnancy were selected as the observation group, and divided into the gestational hypertension group(48 cases), preeclampsia group(34 cases) and severe preeclampsia group(30 cases) according to the severity of the disease. At the same time, 84 pregnant women with normal pregnancy were set as the control group. The serum level of STIM1 was detected by enzyme-linked immunosorbent assay. The flow resistance index(RI), pulsation index(PI), systolic to diastolic velocity ratio(S/D) of uterine artery were measured by ultrasonic diagnostic instrument. The relationship between the serum STIM1 level and hypertensive diseases during pregnancy was analyzed by Pearson correlation analysis. The diagnostic value of serum STIM1 combined with hypertensive diseases during pregnancy in severe preeclampsia were analyzed using ROC curve.
      Results Compared with the control group, the serum STIM1 level, RI, PI and S/D values in the cbsenation group significantly increased(P < 0.05), and the serum STIM1 level, RI, PI and S/D values significantly increased with the increase of disease severity(P < 0.05). The results of Pearson correlation analysis showed that serum STIM1 level was positively correlated with RI, PI and S/D values(r=0.515, 0.620, 0.592, P < 0.05). The results of ROC curve showed that the area under the curve of serum STIM1 combined with artery hemodynamic in the diagnosis of severe preeclampsia was 0.951, which was significantly higher than that of RI, PI, S/D and serum STIM1 alone(P < 0.05).
      Conclusions The hypertensive diseases during pregnancy RI, PI and S/D of pregnant women with gestational hypertension obviously increase, and the serum STIM1 also obviously increases. The diagnostic value of serum STIM1 combined with hypertensive diseases during pregnancy in severe preeclampsia is higher.
