首发精神分裂症病人血清proBDNF、EGF、GFAP水平与病情的相关性研究

    Study on the correlation between the serum levels of proBDNF, EGF, GFAP and condition of first-episode schizophrenia patients

      目的 探讨首发精神分裂症病人血清脑源性神经营养因子前体(proBDNF)、表皮生长因子(EGF)、神经胶质纤维酸性蛋白(GFAP)的表达水平及相关性。
      方法 选择120例首发精神分裂症病人(观察组)及同期90例健康体检者(对照组)作为研究对象。采用酶联免疫吸附法检测血清proBDNF、EGF、GFAP的表达水平。比较2组的proBDNF、EGF、GFAP水平,应用logistic回归分析法分析血清proBDNF、EGF、GFAP与首发精神分裂症严重程度的关系。
      结果 以阳性症状为主的首发精神分裂66例(55.00%),以阴性症状为主的首发精神分裂54例(45.00%)。观察组阴性和阳性症状精神分裂者proBDNF均低于对照组,EGF、GFAP均高于对照组,且阴性症状精神分裂者的proBDNF更低,EGF、GFAP更高,差异均有统计学意义(P < 0.05~P < 0.01)。Pearson相关性分析显示,proBDNF与EGF、GFAP呈负相关,EGF与GFAP呈正相关(P < 0.05)。66例以阳性症状为主的首发精神分裂症病人,轻度组的阳性症状评分、一般病理评分、阳性与阴性症状量表(PANSS)总分、EGF、GFAP均低于中重度组,proBDNF高于中重度组,差异均有统计学意义(P < 0.05~P < 0.01);54例以阴性症状为主的首发精神分裂症病人,轻度组的阴性症状评分、一般病理评分、PANSS总分、EGF、GFAP均低于中重度组,proBDNF高于中重度组,差异均有统计学意义(P < 0.05~P < 0.01)。proBDNF高表达可能是以阳性/阴性症状为主的首发精神分裂症中重度的保护因素,EGF高表达可能是以阳性症状为主的首发精神分裂症中重度的危险因素,GFAP高表达可能是以阳性/阴性症状为主的首发精神分裂症中重度的危险因素(P < 0.05)。
      结论 血清proBDNF在首发精神分裂症中呈低表达,血清EGF、GFAP在首发精神分裂症中呈高表达,proBDNF低表达、EGF和GFAP高表达为PANSS评分高(首发精神分裂症严重)的影响因素,血清proBDNF表达水平下降,血清EGF、GFAP表达水平上升,首发精神分裂症病人可能会出现病情加重现象。

       

      Abstract:
      Objective To explore the expression levels and correlations of serum brain-derived neurotrophic factor precursor(proBDNF), epidermal growth factor(EGF), and glial fibrillary acidic protein(GFAP) in patients with first-episode schizophrenia.
      Methods One hundred and twenty patients with first-episode schizophrenia(observation group) and 90 healthy individuals with physical examinations during the same period(control group) were selected as the research subjects.The expression levels of proBDNF, EGF and GFAP in serum were detected by enzyme-linked immunosorbent assay.The levels of proBDNF, EGF and GFAP between two groups were compared.The logistic regression analysis was used to analyze the relationship between serum proBDNF, EGF, GFAP and severity of first-episode schizophrenia.
      Results There were 66 cases(55.00%) of first-episode schizophrenia mainly with positive symptoms and 54 cases(45.00%) of first-episode schizophrenia mainly with negative symptoms.The proBDNF of schizophrenia patients with negative and positive symptoms in the observation group was lower than that in control group, while the EGF and GFAP were higher than those in the control group.Moreover, the proBDNF of schizophrenia patients with negative symptoms was lower, and the EGF and GFAP were higher, and the differences were statistically significant(P < 0.05 to P < 0.01).The results of Pearson correlation analysis showed that proBDNF was negatively correlated with EGF and GFAP, and EGF was positively correlated with GFAP(P < 0.05).Among the 66 first-episode schizophrenia patients mainly presenting with positive symptoms, the positive symptom score, general pathological score, total positive and negative syndrome scale(PANSS) score, EGF and GFAP in the mild group were lower than those in moderate to severe group, while proBDNF was higher than that in moderate to severe group, and the differences were statistically significant(P < 0.05 to P < 0.01).Among the 54 first-episode schizophrenia patients mainly presenting with negative symptoms, the negative symptom score, general pathological score, total PANSS score, EGF and GFAP in the mild group were lower than those in moderate to severe group, while proBDNF was higher than that in moderate to severe group, and the differences were statistically significant(P < 0.05 to P < 0.01).High expression of proBDNF might be a protective factor for moderate to severe first-episode schizophrenia mainly characterized by positive/negative symptoms, high expression of EGF might be a risk factor for moderate to severe first-episode schizophrenia mainly characterized by positive symptoms, and high expression of GFAP might be a risk factor for moderate to severe first-episode schizophrenia mainly characterized by positive/negative symptoms(P < 0.05).
      Conclusions The serum proBDNF is lowly expressed in first-episode schizophrenia, while serum EGF and GFAP are highly expressed.The low expression of proBDNF and high expression of EGF and GFAP are the influencing factors of high PANSS score(severe first-episode schizophrenia).When the expression level of serum proBDNF decreases, and the expression levels of serum EGF and GFAP increase, patients with first-episode schizophrenia may experience aggravated conditions.
