重度子宫内膜异位症病人外周血Th1/Th17、Tim-3水平表达及意义

    Study on the expression levels and significance of Th1/Th17 and Tim-3 in patients with severe endometriosis

      目的 :探讨重度子宫内膜异位症(EMs)病人外周血辅助T细胞1(Th1)/辅助T细胞17(Th17)、T细胞免疫球蛋白黏蛋白分子3(Tim-3)水平及意义。
      方法 :回顾性分析EMs病人120例临床资料,依据美国生育协会分期标准将病人分为轻度组(Ⅰ ~ Ⅱ期)70例和重度组(Ⅲ ~ Ⅳ期)50例。比较2组病人相关临床资料和子宫内膜异位症生育指数(EFI)及Th1/Th17、Tim-3水平,并绘制ROC曲线评价EFI评分、Th1/Th17、Tim-3对重度EMs的诊断效能,分析EMs严重程度的危险因素。
      结果 :2组病人年龄、体质量指数、产次、孕次、初潮年龄、采集血清样品时的月经周期、病变部位差异均无统计学意义(P > 0.05);重度组病人Tim-3明显高于轻度组(P < 0.01),EFI评分、Th1/Th17均明显低于轻度组(P < 0.01)。ROC曲线分析显示,EFI评分、Th1/Th17、Tim-3预测重度EMs的AUC分别为0.881、0.808、0.867、0.816。多因素logistic回归分析显示,EFI评分<5.927分、Th1/Th17 < 9.775及Tim-3 ≥ 42.023均为重度EMs的独立影响因素(P < 0.05 ~ P < 0.01)。相关分析显示,病人外周血Th1/Th17与EMs严重程度呈明显负相关关系(P < 0.01),Tim-3与EMs严重程度呈明显正相关关系(P < 0.01)。
      结论 :EFI评分<5.927分、Th1/Th17 < 9.775及Tim-3 ≥ 42.023均为重度EMs的独立危险因素,可作为诊断重度EMs的辅助指标,为Ems病人临床病情评估和治疗提供参考。

       

      Abstract:
      Objective To explore the expression levels and significance of peripheral blood helper T cell-1 (Th1)/helper T cell-17 (Th17) and T cell immunoglobulin mucin molecule 3 (Tim-3 in patients with severe endometriosis (EMs).
      Methods The clinical data of 120 EMs patients were retrospective analyzed, and the patients were divided into the mild group (stages I-II, n = 70) and severe group (stages III-IV, n = 50) according to the staging criteria established by the American Fertility Society (AFS). The basic data information, endometriosis fertility index (EFI) and levels of Th1/Th17 and Tim-3 were compared between two groups. The ROC curves were draw to evaluate the effectiveness of EFI score and levels of Th1/Th17 and Tim-3 in diagnosing the severe Ems. The risk factors of severity of Ems were analyzed.
      Results There was no statistical significances in the age, body mass index, birth time, pregnancy time, age of menarche, menstrual cycle at the time of serum sample collection and lesion site between two groups (P > 0.05). The Tim-3 score in the severe group was significantly higher than that in mild group, while the EFI score and Th1/Th17 score were significantly lower than those in mild group (P < 0.01). The results of ROC analysis showed that the AUC of the EFI score, Th1/Th17 and Tim-3 in predicting the severe Ems were 0.881, 0.808, 0.867 and 0.816, respectively. The results of Multivariate logistic regression analysis showed the EFI score < 5.927, Th1/Th17 < 9.775 and Tim-3 ≥ 42.023 were the influencing factors of severe Ems. The results of correlation analysis showed that the peripheral blood Th1/Th17 level was negatively correlated with the severity of EMs, while the Tim-3 level was positively correlated with the severity of Ems (P < 0.05).
      Conclusions The EFI scores < 5.927, Th1/Th17 < 9.775 and Tim-3 ≥ 42.023 are the influencing factors of severe EMs. It can be used as an auxiliary index for the diagnosis of severe EMs, and provide reference for the clinical evaluation and treatment of Ems patients.
