慢性肾脏病3~5期红细胞参数特征分析

    Analysis of red blood cell parameters for patients with CKD at stage 3-5

      目的分析广东省中医院总院住院单纯慢性肾脏病(CKD)3~5期病人的红细胞参数,探讨CKD病人红细胞参数的特征及其临床应用价值。
      方法收集CKD 3~5期各期病人各120例,使用Sysmex XE-5000型全自动血细胞分析仪及配套试剂检测所有研究对象红细胞计数(RBC)、血细胞比容(HCT)、血红蛋白(Hb)、红细胞平均体积(MCV)、红细胞平均血红蛋白(MCH)水平、红细胞平均血红蛋白(MCHC)水平、红细胞分布宽度(RDW),并对参数结果进行统计分析。
      结果同一病程不同性别以及同一性别不同病程的各组病人年龄差异均无统计学意义(P>0.05);在不考虑性别时,不同病程病人的年龄差异有统计学意义(P < 0.05)。男性的MCHC数量在各期均高于女性(P < 0.05~P < 0.01);男性RBC、Hb、HCT数量在CKD 3期、4期均高于女性(P < 0.01),在5期差异无统计学意义(P>0.05);RDW在CKD3期、4期的差异无统计学意义(P>0.05),在5期男性高于女性(P < 0.05);MCV、MCH在各期的性别差异均无统计学意义(P>0.05)。男、女性病人的RBC、Hb和HCT数量均随着分期的升高呈减少趋势(P < 0.01),而RDW呈增加趋势(P < 0.01);MCV和MCH数量在女性病人中随着分期的升高呈减少趋势(P < 0.01),在男性病人中各期之间差异无统计学意义(P>0.05);男、女性的MCHC数量在各期间的差异均无统计学意义(P>0.05)。
      结论RBC、Hb、HCT水平变化与CKD的发展密切相关,根据红细胞参数特征建立的红细胞参数范围可为临床医生评估CKD 3~5期病人的病情提供参考。

       

      Abstract:
      ObjectiveTo investigate the characteristics and clinical significance of red blood cell parameters in patients with chronic kidney disease(CKD) at stages 3-5 with the data from Guangdong Provincial Hospital of Chinese Medicine.
      MethodsOne hundred and twenty cases with CKD in each stage were collected. The red blood cell parameters including red blood cell count(RBC), hematocrit(HCT), hemoglobin(Hb), mean corpuscular volume(MCV), mean corpsular hemoglobin(MCH), mean corpuscular hemoglobin concentration(MCHC) and red cell distribution width(RDW) were obtained from sysmexXE-5000 automatic hematology analyzer.
      ResultsThere was no significant difference in the age of patients with different genders or different courses of the disease(P>0.05). Regardless of gender, the age of patients with different course of disease was statistically significant(P < 0.05). The MCHC of male patients in all stages of CKD was higher than that of female patients(P < 0.05 to P < 0.01. The RBC, Hb and HCT of male patients were higher than those of female patients in CKD stage 3 and 4(P < 0.01), but there was no statistically significance in stage 5(P>0.05). RDW had no statistical difference between stage 3 and 4(P>0.05), but it was higher that in female patients than infe male patients in stages 5(P < 0.05). There was no significant gender difference in MCV and MCH in each stage of CKD(P>0.05). Hb and HCT decreased with the disease progression(P < 0.01), while RDW was just the opposite(P < 0.01). MCV and MCH decreased in female patients as clinical stages increased(P < 0.01), but they had no significant difference in male patients(P>0.05). There was no significant difference in the MCHC between male and female patients(P>0.05).
      ConclusionsThe changes of RBC, Hb and HCT are closely related to the development of CKD. The ranges of red blood cell parameters established based on their characteristics can provide the reference for clinicians to evaluate the condition of the patients with CKD.
