T淋巴细胞亚群及mIPS评分在恶性血液病感染病人中的临床意义

    The clinical significance of T lymphocyte subsets and mIPS scores in infection patients with malignant hematologic diseases

      目的: 探讨T淋巴细胞亚群及感染可能性评分(mIPS评分)在恶性血液病病人感染中的临床意义。
      方法: 选取38例恶性血液病且行化疗治疗的成年病人作为研究对象,根据感染结局分为感染组21例和非感染组17例。比较2组mIPS评分、T淋巴细胞亚群、血常规及生化指标水平。
      结果: 感染组病人mIPS评分、SOFA评分、C反应蛋白水平均明显高于非感染组(P < 0.01)。感染组病人CD3+、CD4+、CD8+水平及中性粒细胞绝对值均低于非感染组(P < 0.05 ~ P < 0.01)。
      结论: 早期检测T淋巴细胞亚群及进行mIPS评分,可对恶性血液病病人感染的发生起到提示及预测作用。

       

      Abstract:
      Objective To explore the clinical significance of T lymphocyte subsets and modified infection probability score (mIPS) in infection patients with malignant hematologic disease.
      Methods A total of 38 adult malignant hematologic disease patients treated with chemotherapy were divided into the infection group (21 cases) and non-infectionb group (17 cases) according to their infection outcomes. The mIPS score, T lymphocyte subsets, blood routine and biochemical index levels were compared between two groups.
      Results The mIPS score, SOFA score and C-reactive protein level in the infection group were significantly higher than those in non-infection group (P < 0.01). The levels of CD3+, CD4+, CD8+ and neutrophils in the infection group were lower than those in non-infection group (P < 0.05 ~ P < 0.01).
      Conclusions Early detection of T lymphocyte subsets and mIPS score can play a role in suggesting and predicting the occurrence of infection in patients with malignant hematologic diseases
