营养风险管理对食管癌内镜黏膜下剥离术后预后分析

    The value of nutritional risk management on the prognostic of esophageal cancer after endoscopic submucosal dissection surgery

      目的: 探讨营养风险管理在食管癌内镜黏膜下剥离术(ESD)术后病人中的应用效果及对预后的影响。
      方法: 选取确诊为早期食管癌并行ESD治疗的60例病人,按照随机数字表法分为观察组(n = 30)和对照组(n = 30),对照组实施术后常规营养干预,观察组实施营养风险管理干预,对比2组病人术后营养指标、术后并发症发生率、营养不良发生率及生活质量。
      结果: 干预后,观察组体质量指数、肱三头肌皮褶厚度及血浆白蛋白水平均优于对照组(P < 0.05~P < 0.01);出院时观察组营养风险评分明显低于对照组(P < 0.01);观察组术后并发症发生率为0%,明显低于对照组的13.33%(P < 0.05);观察组欧洲癌症病人生活质量量表(EORTCQLQ-C30)各维度评分与总评分均明显高于对照组(P < 0.01)。
      结论: 营养风险管理应用于食管癌ESD术后病人能够改善其营养状态,提高其生活质量,降低其营养风险及术后并发症风险,且对于改善其预后具有积极意义。

       

      Abstract:
      Objective To explore the application effects of nutritional risk management in esophageal cance patients treated with endoscopic submucosal dissection (ESD, and its impact on prognosis.
      Methods Sixty early esophageal cancer patients treated with ESD were randomly divided into an the observation group (n = 30) and control group (n = 30) using the random number table method. The control group were given the routine nutritional intervention, while the observation group were given the nutritional risk management. The postoperative nutritional indicators, incidence of postoperative complications, incidence of malnutrition and quality of life were compared between two groups.
      Results After intervention, the BMI index, triceps skinfold thickness and plasma albumin level in the observation group were better than those in control group (P < 0.05). At discharge, the nutritional risk score of the observation group was significantly lower than that of control group (P < 0.05). The incidence rate of postoperative complications in the observation group was 0.00%, whhich was significantly lower than that in control group (13.33%) (P < 0.05). The score of European quality of life scale for cancer patients (EORTCQLQ-C30) in the observation group was significantly higher than that in control group (P < 0.05).
      Conclusions The application of nutritional risk management in patients with esophageal cancer after ESD can improve their nutritional status, improve their quality of life, reduce the risk of nutrition and postoperative complications, and has positive significance for improving their prognosis.
