3D打印导板对上颈椎畸形椎弓根螺钉置入的实验研究

    Experimental study of pedicle screw implantation with 3D printed guide plate for upper cervical deformity

      目的: 评价3D打印导板在上颈椎畸形椎弓根螺钉置入中的安全性与精确性。
      方法: 选取2019年1月至2021年1月蚌埠医学院第二附属医院影像中心的13例上颈椎畸形病人的术前CT数据,等比例设计并打印出个体化上颈椎畸形患者的实体颈椎模型与导板。实验组采用3D打印导板辅助椎弓根螺钉置入;对照组采用术中透视辅助定位徒手置入椎弓根螺钉。术后CT三维重建评价置入螺钉的安全性与精确性。根据螺钉是否发生侧向穿孔,将其分为四类:将Ⅰ类(未穿破)、Ⅱ类(≤2 mm)视为安全放置,Ⅲ类(>2 mm ~ ≤4 mm)、Ⅳ类(>4 mm)视为不安全放置。
      结果: 实验组置入螺钉的安全率98.44%高于与对照组的85.94%(P < 0.01),置入单枚螺钉透视次数(2.69 ± 0.61)次少于对照组(8.66 ± 0.72)次(P < 0.0),用时(11.66 ± 1.17)min短于对照组(23.19 ± 0.43)min(P < 0.01)。
      结论: 3D打印导板对于上颈椎畸形病人椎弓根螺钉的置入相比于徒手置钉方式更安全、更高效、更精确,打破了原有的操作局限性,值得临床推广。

       

      Abstract:
      Objective To evaluate the safety and accuracy of 3D printed guide plate in pedicle screw implantation of upper cervical deformity.
      Methods Preoperative CT data of 13 patients with upper cervical deformity in the Imaging Center of the Second Affiliated Hospital of Bengbu Medical College from January 2019 to January 2021 were selected, and individualized solid cervical spine models and guide plates of patients with upper cervical deformity were designed and printed in equal proportion. The experimental group received 3D printed guide plate assisted pedicle screw implantation and the control group received intraoperative fluoroscopic assisted positioning pedicle screw implantation by hand. Postoperative CT three-dimensional reconstruction was used to evaluate the safety and accuracy of screw implantation. According to whether lateral perforation occurred, the screws were divided into four categories: class Ⅰ (not perforated), class Ⅱ (≤2 mm) as safe placement, classⅢ (> 2 mm ~ ≤4 mm), class Ⅳ (> 4 mm) as unsafe placement.
      Results The safety rates of screw implantation in the experimental group and the control group were 98.44% and 85.94%, respectively (P < 0.05). The number of fluoroscopies for placement of a single screw (2.69 ± 0.61) was less than that of the control group (8.66 ± 0.72) (P < 0.0), and the time taken (11.66 ± 1.17) min was shorter than that of the control group (23.19 ± 0.43) min (P < 0.01).
      Conclusion Compared with freehand screw placement, 3D printed guide plate is safe, more efficient and accurate for pedicle screw implantation in patients with upper cervical deformity, which breaks the original operational limitations and is worthy of clinical promotion.
