    经翼点入路切除蝶骨嵴脑膜瘤16例分析

    Microsurgical treatment of sphenoid ridge meningiomas via pterion approach

    • 摘要: 目的: 总结经翼点入路显微手术切除蝶骨嵴脑膜瘤的经验。方法: 对16例蝶骨嵴脑膜瘤的显微外科手术作回顾性分析。手术入路均为翼点入路,其中蝶骨嵴内1/3者6例,中1/3者4例,外1/3者6例。均在显微镜下行肿瘤切除。结果: 全切除13例,大部切除2例,部分切除1例;其中1例术前已出现脑疝,术后死亡;术后偏瘫1例;余恢复良好。结论: 采用翼点入路,熟练掌握术区显微解剖,保护重要神经和血管,显微镜下切除肿瘤可提高肿瘤切除率,减少术后神经功能障碍。

       

      Abstract: Objective: To sum up the experience of treating sphenoid ridge meningiomas by microsurgery via pterional approach.Methods: Sixteen cases of sphenoid ridge meningiomas were analyzed retrospectively.The surgery was performed via pterion approach.Six of the cases were of medial type tumor,4 cases middle type tumor and 6 cases lateral type tumor.Results: The tumors were totally resented in 13 cases,subtotally resented in 2 cases and partially in 1 cases.One of them died and 1 had hemiplegia.Conclusions: Pterion approach plus micromanipulation and proper preservation of vital vessel and nerves can improve the removal rate of sphenoid ridge meningioma and reduce postoperative nervous malfunction.
