急性缺血性脑卒中病人睡眠质量与执行功能相关性研究

    Sudy on the correlation between sleep quality and executive function in patients with acute ischemic stroke

      目的: 调查急性缺血性脑卒中(acute ischemic stroke,AIS)病人睡眠质量情况及睡眠障碍影响因素,并探讨其与执行功能的相关性。
      方法: 便利抽样法收集AIS病人116例,收集病人的一般资料,采用匹兹堡睡眠质量指数(PSQI)和数字颜色连线测验(CTT)评价病人的睡眠质量及执行功能。根据PSQI最终得分划为睡眠障碍组和非睡眠障碍组,进行二分类logistic回归分析,对AIS病人睡眠质量与执行功能进行相关分析。
      结果: 116名AIS病人PSQI得分为(7.35 ± 4.28)分;执行功能得分,连线Ⅰ型(85.96 ± 39.25)s、连线Ⅱ型(190.53 ± 76.83)s、连线干扰效应(104.57 ± 53.46)s。多因素分析显示,女性比男性会增加发生睡眠障碍的风险(P < 0.05);睡眠障碍组连线Ⅱ型耗时高于睡眠正常组(P < 0.05)。Pearson相关性分析显示,PSQI总分与连线Ⅰ型和连线Ⅱ型均呈正相关关系(P < 0.05)。睡眠时间及睡眠效率与连线Ⅰ型、连线Ⅱ型和连线干扰效应均呈正相关关系,日间功能障碍与连线Ⅰ型、连线Ⅱ型呈正相关关系(P < 0.05)。
      结论: AIS病人睡眠质量处于临界水平,临床上应早期评估AIS病人睡眠质量,识别存在睡眠障碍的病人并尽早干预,为早期预防执行功能提供指导。

       

      Abstract:
      Objective To investigate the sleep quality and influencing factors of sleep disorders in patients with acute ischemic stroke (AIS), and their correlation with executive function.
      Methods A total of 116 patients with AIS were collected by convenience sampling method. The general information of patients was collected. The Pittsburgh Sleep Quality Index (PSQI) and Digital Color Line Test (CTT) were used to evaluate the sleep quality and executive function of patients. According to the final PSQI score, the patients were divided into the sleep disorder group and non-sleep disorder group. Binary logistic regression analysis was used to analyze the correlation between sleep quality and executive function of AIS patients.
      Results The PSQI score of 116 AIS patients was (7.35 ± 4.28), The PSQI score of 116 AIS patients was (7.35 ± 4.28) points. The scores of executive function were as follows: Type I connection (85.96 ± 39.25)s, Type II connection (190.53 ± 76.83)s, and interference effect connection (104.57 ± 53.46)s. The results of multivariate analysis showed that women had an increased risk of sleep disorders than men (P < 0.05); The time consumed for connecting type II in the sleep disorder group was higher than that in normal sleep group (P < 0.05). The results of Pearson correlation analysis showed that the total score of PSQI was positively correlated with both line type I and line type II (P < 0.05). The sleep duration and sleep efficiency were positively correlated with connection type I, connection type II and connection disturbance effect, and the daytime dysfunction was positively correlated with connection type I and connection type II (P < 0.05).
      Conclusions The sleep quality in patients with AIS is at a critical level, and the early assessment of sleep quality in patients with AIS should be performed clinically to identify patients with sleep disorders and intervene as early as possible to provide guidance for early prevention of executive function.
