脂蛋白(a)在心肌梗死自然过程的作用

    The function of lipoprotein(a) in the natural course of myocardial infarction

    • 摘要: 目的:研究脂蛋白(a)Lp(a)与未接受再灌注治疗心肌梗死(myocardial infarction,MI)患者冠状动脉残余病变的关系,探讨Lp(a)在MI自然发展过程的作用。方法:对124例未接受再灌注治疗的MI患者进行冠状动脉造影。造影前抽取清晨空腹静脉血,检测Lp(a)、血糖、总胆固醇(TC)和甘油三酯(TG)。将124例患者分为低Lp(a)组(300 mg/L)79例,高Lp(a)组(300 mg/L)45例;糖尿病组72例,非糖尿病组52例;低TC组(5.69 mmol/L)96例,高TC组(5.69 mmol/L)28例;低TG组(1.69 mmol/L)79例,高TG组(1.69 mmol/L)45例。比较各组冠状动脉残余病变狭窄程度和TIMI分级。结果:高Lp(a)组冠状动脉残余病变狭窄程度显著高于低Lp(a)组(P0.01),TIMI分级低于低Lp(a)组(P0.05),而其他组差异均无统计学意义(P0.05)。结论:对于未接受再灌注治疗的MI患者,高水平的血浆Lp(a)通过促进血栓形成加重冠状动脉残余病变狭窄程度;临床监测血浆Lp(a)有利于观察MI病程,可作为评估溶栓效果和预后的重要指标。

       

      Abstract: Objective:To investigate the influence of lipoprotein(a) on the residual lesion stenosis of the infarction-related arteries in acute myocardial infarction(MI) patients in whom reperfusion therapy was not performed.To explore the function of Lp(a) in the natural course of MI.Methods:One hundred and twenty-four MI patients who not given reperfusion therapy were underwent coronary angiography in chronic stage.Morning fasting blood was collected and Lp(a),blood sugar,total cholesterol(TC) and triglycerides(TG) were measured.Residual stenosis and TIMI classification were compared between the low Lp(a) group(300 mg/L) and the high Lp(a) group(300 mg/L),non-DM and DM,low-TC(5.69 mmol/L) and high-TC(5.69 mmol/L),low-TG(1.69 mmol/L) and high-TG(1.69 mmol/L) groups.Results:Residual stenosis was more severe in high Lp(a) level group than in low Lp(a) level group(P0.01).TIMI classification in high Lp(a) level group is lower(P0.05).Conclusions:These findings suggest that elevated Lp(a) level aggravate residual lesion stenosis of the infarction-related arteries acute MI patients in whom reperfusion therapy is not performed.The measurement of serum Lp(a) level is propitious to observe the nature course of MI,it might be diagnosable and prognostic factor to evaluate the effect of thrombolytic therapy.
