FABP2基因多态性与2型糖尿病及其非糖尿病一级亲属代谢的关系

    The correlation between FABP2 polymorphism and the metabolism of type 2 diabetic patients and their non-diabetic first-degree relatives

    • 摘要: 目的:探讨小肠脂肪酸结合蛋白基因(FABP2)多态性与2型糖尿病(T2DM)及其非糖尿病一级亲属(NDR)代谢的关系。方法:选择80例T2DM患者及其NDR 84例和正常对照组(NC组)60例,应用聚合酶链反应方法对3组的小肠脂肪酸结合蛋白基因54密码子的HhaⅠ酶切位点进行限性片段长度多态性进行分析。结果:3组中均存在FABP2 HhaⅠ多态性位点,有FABP2 Ala54Ala、Ala54Thr和Thr54Thr多态片段(等位基因频率Ala为0.67,Thr为0.33)。与NC组比较,T2DM组的胰岛素抵抗指数(HOMA-IR指数)、空腹胰岛素均明显增高(P0.01),胰岛素敏感指数(ISI)显著降低(P0.01);三酰甘油、总胆固醇、低密度脂蛋白明显增高(P0.01),高密度脂蛋白明显降低(P0.01)。T2DM组中,FABP2 Thr(-)与FABP2 Thr(+)基因型(FABP2 Ala54Ala与Ala54Thr及Thr54Thr基因型)患者比较,后者的空腹胰岛素、HOMA-IR指数、空腹血糖、三酰甘油水平、低密度脂蛋白均明显增高(P0.01),而ISI和高密度脂蛋白均降低(P0.01)。NDR组中FABP2 Thr(-)与FABP2Thr(+)基因型患者比较,后者的HOMA-IR指数、三酰甘油水平均明显增高(P0.01和P0.05),而ISI显著降低(P0.01)。结论:蚌埠地区汉族人存在FABP2多态性。与NC组比较,T2DM组、NDR组中存在胰岛素抵抗及脂代谢异常。T2DM组、NDR组中,FABP2 Thr(+)基因型与胰岛素抵抗相关,是脂代谢异常的影响因素之一。

       

      Abstract: Objective: To explore the correlation between human intestinal fatty acid binding protein 2(FABP2) polymorphism and the metabolism of type 2 diabetic patients and their non-diabetic first-degree relatives.Methods: Two hundred and twenty-four patients with type 2 diabetes mellitus(T2DM) and their 84 non-diabetic first degree relatives(NDR),60 healthy person were divided into the T2DM,NDR and NC group,respectively.The restricted fragment length polymorphisms of Hha Ⅰ enzyme site in codon 54 of the intestinal FABP gene of three groups were analysed by PCR.Results: The polymorphic sites of FABP2 HhaⅠincluding Ala54Ala and Ala54Thr of FABP2 and Thr54Thr polymorphism fragment were found in three groups.The allele freqencies of Ala54 and thr54 were 0.67 and 0.33,respectively.Compared with NC group,the insulin resistance(IR) index,fasting insulin,and the levels of triglyceride(TG),total cholesterol(TC) and low density lipoprotein(LDL) increased significantly(P 0.01),the insulin sensitivity index and the level of high density lipoprotein(HDL) decreased significantly in T2DM group(P 0.01).Compared with patients with FABP2 Thr(-) genotype of T2DM group,the FINS,IR index,and the levels of fasting blood glucose(FBG),TG and LDL increased significantly(P 0.01),the insulin sensitivity index and the level of HDL decreased significantly in patients with FABP2 Thr(+) genotype(FABP2 Ala54Ala,Ala54Thr and Thr54Thr) of T2DM group(P 0.01).Compared to patients with FABP2 Thr(-) genotype of NDR group,the IR index and the level of TG increased significantly(P 0.01 and P 0.05),the insulin sensitivity index decreased significantly in patients with FABP2 Thr(+) genotype of T2DM group(P 0.01).Conclusions: The Han has FABP2 polymorphism in Bengbu.The IR and abnormal lipid metabolism exist in T2DM and NDR group.The FABP2 Thr(+) genotypes in T2DM and NDR groups are related to IR,which is one factor of abnormal lipid metabolism.
