    右美托咪定对硬膜外麻醉期间寒战的预防

    Dexmedetomidine given by total intra venous anaesthesia inject mode can prevent the incidence of shivering during epidural anesthesia

    • 摘要: 目的：观察全凭静脉麻醉注射(TIVAI)模式输注右美托咪定预防低位硬膜外麻醉期间寒战的临床效果。方法：将ASAⅠ～Ⅱ级、行低位硬膜外麻醉的60例患者按单盲随机原则均分成观察组和对照组。观察组采用TIVAI模式输注右美托咪定，诱导量为0.8g/kg，在15min内输注完，维持量为0.2g.kg-1.h-1，持续到手术终止；对照组以同样形式输注0.9%氯化钠注射液。结果：对照组寒战发生率及发生寒战的程度均明显高于观察组(P0.01)；对照组在手术结束时诉麻醉期间有不舒适感觉者明显多于观察组(P0.01)，其中需要干预缓解者占36.7%。结论：TIVAI模式输注右美托咪定可以有效地预防低位硬膜外麻醉期间寒战的发生，患者平静、舒适，符合现在舒适化医疗要求。

       

      Abstract: Objective:To observe the clinical effects of dexmedetomidine given by total intra venous anaesthesia igject(TIVAI)mode in preventing shivering during low epidural anesthesia.Methods:Sixty ASA Ⅰ or Ⅱ patients receiving low epidural anesthesia were randomly divided into observation group and control group by single blind method.In the observation group dexmedetomidine was given by TIVAI mode at a concentration of 0.8 g/kg within 15 minutes,and then dexmedetomidine was infused consistently at the rate of 0.2 gkg-1h-1until the end of operation;while in control group,0.9% sodium chloride injection was given by TIVAI mode instead.Results:The incidence and the extent of shivering in the control group was significantly higher than that in the observation group(P0.01);after the operation,more patients in the control group complained of discomfort during operation(P0.01).And 36.7% of them was not relieved until intervention was applied.Conclusions:Dexmedetomidine given by TIVAI mode can effectively prevent the incidence of shivering during low epidural anesthesia and make the patients feel peaceful and comfortable,which adapts to the current requirements of comfortable medical treatment.
