血清FOXO1和PPAR-γ水平在下肢深静脉血栓诊断中的应用价值

    The value of serum levels of FOXO1 and PPAR-γ in the diagnosis of deep venous thrombosis of lower limbs

      目的: 探讨血清叉头转录因子O1(FOXO1)和过氧化物酶体增殖物激活受体-γ(PPAR-γ)水平在下肢深静脉血栓(DVT)诊断中的应用价值。
      方法: 选取下肢障碍病人132例,按照数字减影血管造影(DSA)诊断结果,将病人分为DVT组87例和非DVT组45例,另选取同期体检健康者132名作为对照组。分析DVT发生的影响因素及DVT病人血清FOXO1和PPAR-γ表达水平的相关性,并采用ROC曲线分析血清FOXO1和PPAR-γ水平对DVT的诊断价值。
      结果: 3组凝血酶原时间、活化部分凝血酶原时间、血小板计数和FOXO1、PPAR-γ表达水平间差异均有统计学意义(P < 0.01)。DVT病人血清FOXO1和PPAR-γ表达水平呈负相关关系(P < 0.05)。高血压史、凝血酶原时间<8 s、活化部分凝血酶原时间<27 s、血小板计数≥300 × 109/L和高FOXO1均为DVT的独立危险因素,而PPAR-γ为其保护因素(P < 0.05 ~ P < 0.01)。血清FOXO1、PPAR-γ以及二者联合检测诊断DVT的AUC分别为0.745、0.657、0.831,二者联合检测优于单独检测(P < 0.01)。
      结论: 血清FOXO1、PPAR-γ表达水平与DVT发生密切相关,二者联合检测对DVT具有较好诊断价值,可用于辅助诊断DVT。

       

      Abstract:
      Objective To investigate the clinical value of serum levels of forkhead box protein O1 (FOXO1) and peroxisome proliferator-activated receptor-γ (PPAR-γ) in the diagnosis of deep venous thrombosis (DVT) of lower limbs.
      Methods A total of 132 patients with lower limb disorders were selected. According to the diagnosis results of digital subtraction angiography (DSA), the patients were divided into the DVT group (87 cases) and non DVT group (45 cases). Additionally, 132 healthy physical examiners during the same period were selected as the control group. The influencing factors of DVT and correlation between serum FOXO1 and PPAR-γ expression levels in DVT patients were analyzed, and the ROC curve was used to analyze the diagnostic value of serum FOXO1 and PPAR-γ levels for DVT.
      Results There were statistically significant in the PT, APTT, platelet counts and levels of FOXO1 and PPAR-γ among three groups (P < 0.05). There was a negative correlation between serum FOXO1 and PPAR-γ expression levels in DVT patients (r = –0.750, P < 0.05). The history of hypertension, prothrombin time (PT) < 8 s, activated partial prothrombin time (APTT) < 27s, platelet count ≥ 300 × 109/L and high FOXO1 were the independent risk factors of DVT, while the PPAR-γ was a protective factor of DVT (P < 0.05 ~ P < 0.01). The area under curve (AUC) of serum FOXO1, PPAR-γ and their combined detection in the diagnosis of DVT were 0.745, 0.657 and 0.831, respectively, the combined detection of both was superior to the individual detection of serum FOXO1 and PPAR-γ (P < 0.01).
      Conclusions The expression levels of serum FOXO1 and PPAR-γ are closely related to the occurrence of DVT. The combined detection of FOXO1 and PPAR-γ has good diagnostic value for DVT, and can be used to assist in the diagnosis of DVT.
