急性重度有机磷农药中毒并呼吸衰竭53例救治体会

Treatment of patients with severe organophosphorus pesticide intoxication complicated with respiratory failure: a report of 53

摘要: 目的:探讨急性重度有机磷农药中毒(AOPP)合并呼吸衰竭救治方法。方法:53例在彻底洗胃和一般内科常规治疗基础上,采用突击量氯磷定疗法、尽早阿托品化、机械通气(MV)等综合抢救措施。结果:抢救成功44例,成功率为83%,自动放弃5例,死亡4例。结论:早期彻底洗胃、合理应用阿托品或盐酸戊乙奎醚、对昏迷患者先气管插管后胃管洗胃、早期、合理使用MV等救治方法配合恰当,可提高疗效。

Abstract: Objective: To sum up the experience in rescuing patients with severe organophosphorus pesticide intoxication combined with respiratory failure.Methods: After thorough gastric lavage and general medical conventional therapy,53 patients with severe organophosphorus pesticide intoxication were treated by flushing dose of pralidoxime chloride,atropinization,mechanical ventilation,and other comprehensive emergency medical therapies.Results: Forty-four patients survived(83%).Five cases gave up.Conclusions: Timely and thorough gastric lavage,proper application of atropine or changtuoling,rational use of tracheal intubation,mechanical ventilation and other techniques can improve the efficacy.
