同型半胱氨酸和C反应蛋白与2型糖尿病大血管病变的关系研究

    The relationship between the levels of homocysteine and C-reactive protein and macroangiopathy in type 2 diabetes mellitus

    • 摘要: 目的:探讨血浆同型半胱氨酸(Hcy)和C反应蛋白(CRP)与2型糖尿病大血管病变的关系。方法:免疫比浊法检测正常对照组(30名)、单纯糖尿病组(32例)及糖尿病大血管病变组(31例)血浆Hcy和CRP水平,观察其水平变化与糖尿病大血管病变的关系。结果:糖尿病大血管病变组和单纯糖尿病组血浆Hcy和CRP均明显高于正常对照组(P<0.01),单纯糖尿病组Hcy和CRP均显著低于糖尿病大血管病变组(P<0.01);logistic回归分析显示收缩压、舒张压、糖化血红蛋白、三酰甘油、总胆固醇、高密度脂蛋白、Hcy和CRP均与动脉粥样硬化的发生相关。结论:2型糖尿病大血管病变患者血浆Hcy 和CRP水平明显升高,消除动脉粥样硬化的危险因素如减轻体质量、控制血糖、纠正脂代谢紊乱、增强胰岛素敏感性、降血压等综合治疗可预防和延缓动脉粥样硬化的发生。

       

      Abstract: Objective: To investigate the relationship between the levels of homocysteine(Hcy) and C-reactive protein(CRP) and macroangiopathy in type 2 diabetes mellitus.Methods: The levels of plasma Hcy and CRP in control group(30 cases),simple diabetes mellitus group(32 cases) and diabetic macroangiopathy group(31 cases) were detected using immunoturbidimetry.The relationship between the levels of Hcy and CRP and diabetic macroangiopathy was analyzed.Results: The levels of plasma Hcy and CRP in simple diabetes mellitus and diabetic macroangiopathy group were significantly higher than those in control group(P<0.01),the levels of plasma Hcy and CRP in simple diabetes mellitus group were significantly lower than those in and diabetic macroangiopathy group(P<0.01).Logistic regression analysis showed that the levels of systolic blood pressure,diastolic blood pressure,HbA1c,triglyceride,total cholesterol,high density lipoprotein,Hcy and CRP were correlated to artherosclerosis.Conclusions: The levels of plasma Hcy and CRP in type 2 diabetic patients with macroangiopathy increased significantly.The treatment with losing body mass,controlling blood glucose,correcting lipid metabolism disorder,increasing insulin sensitivity reducing blood pressure can prevent and delay the occurrence of artherosclerosis.
