高危型HPV感染病人阴道微生态失调与免疫指标相关性

    Study on the correlation between vaginal microecological disorder and immune indexes in patients with high-risk HPV infection

      目的: 探讨宫颈高危型人乳头瘤病毒(HR-HPV)感染病人阴道微生态失调与免疫指标的关系。
      方法: 选取接受HR-HPV感染治疗病人147例作为观察组,根据有无阴道微生态失衡再分为失调组93例和正常组54例,另选取同期健康体检女性140名作为对照组。检测各组血清中白细胞介素(IL)-6、IL-10、肿瘤坏死因子α(TNF-α)水平和免疫球蛋白(Ig)A、IgM、IgG及血清中T细胞亚群CD8+、CD3+、CD4+、CD4+/CD8+表达水平,分析免疫指标与阴道微生态失调的相关性。
      结果: 观察组和对照组菌群密度、多样性和pH、H2O2、白细胞酯酶、Nugent评分、乳酸杆菌比例及阴道微生态失衡情况差异均有统计学意义(P < 0.01),2组清洁度差异无统计学意义(P > 0.05)。失调组病人血清TNF-α、IL-6、IL-10、CD8+、IgA、IgG、IgM水平均较正常组和对照组升高(P < 0.05),CD3+、CD4+、CD4+/CD8+均较正常组和对照组降低(P < 0.05)。HR-HPV感染病人Nugent评分与TNF-α、IL-6、IL-10、CD8+、IgA、IgG、IgM均呈明显正相关关系(P < 0.01),与CD3+、CD4+、CD4+/CD8+均呈明显负相关关系(P < 0.01)。
      结论: HR-HPV感染病人血清免疫指标高表达,其与阴道微生态失调相关。

       

      Abstract:
      Objective To explore the correlation between vaginal microecological disorder and immune indexes in patients with high-risk human papillomavirus (HR-HPV) infection.
      Methods A total of 147 patients receiving HR-HPV infection treatment were selected as the observation group, and which were further divided into the disorder group (93 cases) and the normal group (54 cases) according to the presence or absence of vaginal microecological imbalance, and 140 healthy women in the same period were selected as the control group. The serum levels of interleukin (IL) -6, IL-10, tumor necrosis factor α (TNF-α), immunoglobulin (Ig) A, IgM, IgG and T cell subsets CD8+, CD3+, CD4+, CD4+/CD8+ in each group were detected, and the correlation between immune indexes and vaginal microecological dysbiosis was analyzed.
      Results The differences of the bacterial density, diversity, pH, H2O2, leucocyte esterase, Nugent score, lactobacillus ratio and vaginal microecological imbalance between th observation group and control group were statistically signifciant (P < 0.01), and there was no statistical significance in the cleanliness between two groups (P > 0.05). The serum levels of TNF-α, IL-6, IL-10, CD8+, IgA, IgG and IgM in the disorder group were higher than those in normal group and control group (P < 0.05), while the levels of the CD3+, CD4+and CD4+/CD8+in the disorder group were lower than those in normal group and control group (P < 0.05). The Nugent score was positively correlated with TNF-α, IL-6, IL-10, CD8+, IgA, IgG and IgM in HR-HPV infected patients (P < 0.01), and negatively correlated with CD3+, CD4+and CD4+/CD8+(P < 0.01).
      Conclusions The serum immune indexes of patients with HR-HPV infection are highly expressed, which is closely related to the vaginal micro-ecological disorders.
