CRP/ALB、PLR、NLR与类风湿关节炎病人疾病活动度指标及DAS28评分的关系

    The relationship between CRP/ALB, PLR, NLR and disease activity indicators as well as DAS28 score in patients with rheumatoid arthritis

      目的: 探讨C反应蛋白与白蛋白比值(CRP/ALB)、血小板淋巴细胞比值(PLR)、中性粒细胞淋巴细胞比值(NLR)与类风湿关节炎(RA)病人疾病活动度指标及28个关节疾病活动度(DAS28)评分的相关性,评估其在RA病情评估中的价值。
      方法: 选取121例RA病人,依据DAS28–ESR评分分为缓解组(15例)、低度活动组(25例)、中度活动组(52例)及高度活动组(29例)。采集病人外周静脉血,获取CRP/ALB、PLR、NLR、类风湿因子(RF)、红细胞沉降率(ESR)水平,通过单因素方差分析、Spearman相关分析、多因素logistic回归分析及受试者工作特征(ROC)曲线评估指标的临床意义。
      结果: 随着疾病活动度升高,CRP/ALB、PLR、NLR、RF及ESR水平呈逐步递增趋势(P < 0.01)。Spearman相关分析显示,CRP/ALB、PLR、NLR与RF、ESR及DAS28评分均呈正相关(P < 0.01)。多因素logistic回归分析表明,CRP/ALB(OR = 7.368)、PLR(OR = 1.014)和NLR(OR = 2.625)均是RA病人中高度活动度的独立危险因素(P < 0.01)。ROC曲线分析显示,三者联合预测RA中高度活动度的曲线下面积为0.878(95%CI:0.816 ~ 0.939),灵敏度79.00%,特异度80.00%,约登指数0.590。
      结论: CRP/ALB、PLR、NLR与RA疾病活动度密切相关,是评估病情的有效指标,三者联合应用能显著提升对中高度疾病活动度的预测价值,具有重要的临床参考意义。

       

      Abstract:
      Objective To explore the correlations between the ratio of C-reactive protein to albumin (CRP/ALB), platelet-lymphocyte ratio (PLR), neutrophil-lymphocyte ratio (NLR) and disease activity index, 28-joint disease activity (DAS28) score of patients with rheumatoid arthritis (RA), and evaluate their value in the assessment of RA conditions.
      Methods A total of 121 RA patients were selected, and divided into the remission group (15 cases), low activity group (25 cases), moderate activity group (52 cases) and high activity group (29 cases) according to the AS28-ESR score. Peripheral venous blood was collected from the patients to detected the levels of CRP/ALB, PLR, NLR, rheumatoid factor (RF) and erythrocyte sedimentation rate (ESR). The clinical significance of the indicators were evaluated through one-way analysis of variance, Spearman correlation analysis, multivariate logistic regression analysis and receiver operating characteristic (ROC) curve.
      Results With the increase of disease activity, the levels of CRP/ALB, PLR, NLR, RF and ESR showed a gradually increasing trend (P < 0.01). Spearman correlation analysis showed that the CRP/ALB, PLR and NLR were positively correlated with RF, ESR, and DAS28 scores (P < 0.01). Multivariate logistic regression analysis indicated that the CRP/ALB (OR = 7.368), PLR (OR = 1.014) and NLR (OR = 2.625) were all independent risk factors of high activity in RA patients (P < 0.01). ROC curve analysis showed that the area under the curve for the combined prediction of high activity in RA by the three was 0.878 (95%CI: 0.816–0.939), with a sensitivity of 79.00%, a specificity of 80.00% and a Youden index of 0.590.
      Conclusions CRP/ALB, PLR, and NLR are closely related to the disease activity of RA, and effective indicators for evaluating the condition. The combined application of the three can significantly enhance the predictive value for moderate to high disease activity, and has important clinical reference significance.
