    缺血性脑血管病与踝臂指数的相关性研究

    Correlation study of ischemic cerebrovascular disease and ankle brachial index

    • 摘要: 目的:对缺血性脑血管病(ICVD)与踝臂指数(ABI)的相关性进行探讨。方法:选取住院治疗且行颈动脉彩超检查和经颅彩色多普勒超声检查的ICVD患者120例,所有患者均行ABI测量和常规生化检查。按血管狭窄程度及病变血管数量对患者进行分组,并进行中国缺血性脑卒中亚型分型,比较颅内动脉狭窄程度、病变数量、部位以及不同类型脑卒中与ABI的关系,随访1年,记录心脑血管事件的发生情况,分析心脑血管事件与ABI的相关关系。结果:随着狭窄支数及程度增加,ABI异常率均呈增加趋势,ABI均值呈递减趋势(P<0.05~P<0.01);穿支动脉病变组的ABI值显著高于大动脉粥样硬化组(P<0.01);ABI正常组心血管事件总发生率为12.5%,明显低于ABI异常组的48.8%(P<0.01)。结论:ICVD颅内动脉狭窄严重程度、病变支数、病变部位及预后与ABI相关,ABI对重度颅内动脉狭窄、3支以上病变及大动脉粥样硬化疾病分型及心血管事件发生情况有较高的预测价值。

       

      Abstract: Objective: To study the correlation between ischemic cerebrovascular disease(ICVD) and ankle brachial index(ABI).Methods: The ABI measurement and routine biochemical tests were implemented in 120 ICVD patients diagnosed by arotid artery ultrasonography and transcranial color doppler(TCD) ultrasound.The patients were divided into the different groups according to the vascular stenosis degree,blood vessel number of lesions,and different Chinese ischemic stroke subtypes(CISS).The relationship between the intracranial arterial stenosis,number and location of lesions and different types of stroke,and ABI were compared.The cardiovascular events were observed during the period of 1-year following-up,and the correlation between cardiovascular events and ABI were analyzed.Results: With the number and extent of stenosis increasing,the abnormal rate of ABI increased,and the average value of ABI decreased(P<0.05 to P<0.01).The ABI value in single artery lesion group was significantly higher than that in large artery atherosclerosis group(P<0.01).The total incidence of cardiovascular event in normal ABI group(12.5%) was significantly lower than that in abnormal ABI group(48.8%)(P<0.01).Conclusions: The intracranial arterial ischemic degree,number and location of lesion and prognosis are related to the ABI.The ABI has high predictive value in severe intracranial artery stenosis,more than 3 branches lesions,large artery atheroscleosis disease classification and cardiovascular event.
