    中性粒细胞/淋巴细胞比值和胱抑素C对2型糖尿病肾病的早期诊断价值

    Early diagnostic value of the neutrophil to lymphocyte ratio and cystatin C in type 2 diabetic nephropathy

    • 摘要: 目的:探讨全血中性粒细胞/淋巴细胞比值(NLR)和血清胱抑素C(CysC)在2型糖尿病早期肾损害的预测价值。方法:随机选取2型糖尿病病人182例,根据尿微量白蛋白/肌酐比值(ACR)分为单纯糖尿病组(ACR<3.4 mg/mmol)和早期糖尿病肾病组(3.4 mg/mmol ≤ ACR<33.9 mg/mmol),比较2组间性别、年龄、糖尿病病程以及全血NLR、血清尿素(UREA)、肌酐(CREA)、尿酸(UA)、CysC水平是否存在差异,并进行ROC曲线分析和logistic回归分析。结果:2组间性别、糖尿病病程以及血清UREA、CREA、UA水平比较差异均无统计学意义(P>0.05),而早期糖尿病肾病组年龄和全血NLR、血清CysC水平高于单纯糖尿病组(P<0.05~P<0.01)。血清CysC对早期糖尿病肾病的诊断价值高于年龄和全血NLR,三者ROC曲线下面积(AUC)分别为0.831、0.678和0.638,灵敏度分别为70.59%、92.86%和92.86%,特异度分别为90.00%、41.27%和36.51%,阴性预测值分别为84.36%、92.85%和91.99%,阳性预测值分别为80.02%、41.31%和39.43%。血清CysC是早期糖尿病肾病的危险因素,OR为266.080。结论:年龄和全血NLR可作为早期糖尿病肾病的筛查指标,而血清CysC对早期糖尿病肾病的预测价值较高,是其独立危险因素。

       

      Abstract: Objective:To investigate the predictive value of the neutrophil to lymphocyte ratio(NLR) and cystatin C(CysC) in early renal damage of type 2 diabetes patients.Methods:One hundred and eighty-two type 2 diabetes patients were divided into the simple diabetes groupalbumin to creatinine(ACR)<3.4 mg/mmol and early diabetic nephropathy group(3.4 ≤ ACR<33.9 mg/mmol) according to the urinary ACR ratio.The levels of gender,age,duration of diabetes,whole blood NLR,serum urea(UREA),creatinine(CREA),uric acid(UA) and CysC between two groups were compared,and analyzed using the receiver operating curve(ROC) and logistic regression.Results:The differences of the gender,duration of diabetes,and serum levels of UREA,CREA and UA between two groups were not statistically significant(P>0.05),but the age,whole blood NLR and serum level of CysC in early diabetic nephropathy group were higher than those in simple diabetic group(P< 0.05 to P<0.01).The diagnostic value of serum CysC in early diabetic nephropathy was higher than that of age and whole blood NLR.The area under ROC curve of CysC,age and whole blood NLR were 0.831,0.678 and 0.638,respectively,the sensitivities of CysC,age and whole blood NLR were 70.59%,92.86% and 92.86%,respectively,the specificities of CysC,age and whole blood NLR were 90.00%,41.27% and 36.51%,respectively,the negative predictive values of CysC,age and whole blood NLR were 84.36%,92.85% and 91.99%,respectively,and the positive predictive values of CysC,age and whole blood NLR were 80.02%,41.31% and 39.43%,respectively.Serum CysC was a risk factor in early diabetic nephropathy,and the odds ratio of which was 266.080.Conclusions:Age and whole blood NLR can be set as a screening index for early diabetic nephropathy.The serum CysC has highly predictive value,which is an independent risk factor in early diabetic nephropathy.
