    乙酰半胱氨酸联合低分子肝素对特发性间质性肺炎病人肺功能及血气的影响

    Effect of acetylcysteine combined with low-molecular-weight heparin on pulmonary function and blood gas in patients with idiopathic interstitial pneumonia

    • 摘要:
      目的分析乙酰半胱氨酸联合低分子肝素对特发性间质性肺炎(ⅡP)病人肺功能及血气的影响。
      方法选取98例ⅡP病人分为观察组和对照组,各49例,其中对照组给予常规治疗,观察组在此基础上给予乙酰半胱氨酸联合低分子肝素治疗,治疗时间为2个月。比较2组ⅡP病人治疗前后肺功能指标、动脉血气分析和临床症状的改变。
      结果治疗2个月后,观察组ⅡP病人呼吸困难、咳嗽咳痰、肺部Velcro音评分显著低于治疗前(P < 0.01);观察组治疗后用力肺活量占预计值百分比、第1秒用力呼气容积占预计值百分比、肺活量显著优于治疗前(P < 0.01),且显著高于对照组(P < 0.01);2组ⅡP病人治疗后动脉血氧分压显著高于治疗前(P < 0.01),动脉血二氧化碳分压显著低于治疗前(P < 0.01)。
      结论乙酰半胱氨酸联合低分子肝素治疗有利于改善ⅡP病人的肺功能,提高动脉血氧分压,具有确切的临床疗效。

       

      Abstract:
      ObjectiveTo analyze the effects of acetylcysteine combined with low-molecular-weight heparin on pulmonary function and blood gas in patients with idiopathic interstitial pneumonia(ⅡP).
      MethodsNinety-eight patients with ⅡP were randomly divided into the observation group and control group(49 cases in each group).The control group was treated with conventional method, and the observation group was additionally treated with acetylcysteine combined with low-molecular-weight heparin on the basis of the control group for 2 months.The changes of pulmonary function index, arterial blood gas analysis and clinical symptoms between two groups were compared before and after treatment.
      ResultsAfter 2 months of treatment, the scores of dyspnea, cough, expectoration and lung Velcro sound in observation group were significantly lower than those before treatment(P < 0.01), and significantly higher than those in control group(P < 0.01).After treatment, the arterial partial pressure of oxygen in two groups were significantly higher than that before treatment(P < 0.01), and the arterial partial pressure of carbon dioxide in two groups were significantly lower than that before treatment(P < 0.01).
      ConclusionsThe treatment of acetylcysteine combined with low-molecular-weight heparin is beneficial to improve the pulmonary function of patients with ⅡP and improve the arterial partial pressure of oxygen, and has good clinical effects.
