    Imbalance of Th17/Treg in the pathogenesis of bronchiolitis

    • 摘要:
      目的探讨毛细支气管炎患儿外周血辅助T淋巴细胞17(Th17)/调节性T淋巴细胞(Treg)和相关细胞因子的水平与本病发展的关系。
      方法选取毛细支气管炎患儿83例为病例组,分为轻症组(42例)、中症组(21例)和重症组(20例),同期健康体检的儿童30名为对照组。采用流式细胞学检测外周血辅助T细胞17(Th17)和调节性T细胞(Treg)占CD4+T细胞的比值,应用ELISA方法检测血清白细胞介素6(IL-6)、IL-10、IL-17和IL-35的浓度,比较各组间上述指标差异。
      结果病例组Th17细胞比值及其分泌的促炎因子IL-17、IL-6较对照组明显增高(P<0.05),而Treg细胞比值及其分泌的抑炎因子IL-10、IL-35则明显降低,重症组愈加显著(P<0.05)。Pearson相关分析显示Th17与Th17/Treg、IL-17、IL-6呈正相关;Treg与IL-10、IL-35呈正相关,与Th17/Treg负相关;Th17/Treg、Th17和IL-17与IL-35负相关(P<0.05~P<0.01)。logistic回归分析显示Th17/Treg、IL-7和IL-6是重症毛细支气管炎的危险因素(P<0.05~P<0.01)。
      结论Th17/Treg失衡在毛细支气管炎发病中起着重要的作用,Th17/Treg、IL-17和IL-6是重症毛细支气管炎的危险因素。

       

      Abstract:
      ObjectiveTo investigatethe the levels of helper T lymphocyte 17(Th17)/regulatory T lymphocyte(Treg) and related cytokines in children with bronchiolitis, and its correlation with the progress of disease.
      MethodsEighty-three children with bronchiolitis were divided into the light disease group(42 cases), medium disease group(21 cases) and severe disease group(20 cases), and 30 healthy physical examination children at the same time were set as the control group.The ratio of Th17 and Treg to CD4+T cells were detected using flow cytometry.The serum concentrations of interleukin(IL)-6, IL-10, IL-17 and IL-35 were detected using ELISA, and the differences of above indicators were analyzed among four groups.
      ResultsThe ratio of Th17 cells and levels of pro-inflammatory factors of IL-17 and IL-6 in case group were significantly higher than those in control group(P<0.05), while the ratio of Treg cells and production of anti-inflammatory factors of IL-10 and IL-35 in case group were significantly lower than those in control group(P<0.05), especially in the severe group.The results of Pearson correlation analysis showed that the level of Th17 was positively correlated with levels of Th17/Treg, IL-17 and IL-6, and the level of Treg was positively correlated with levels of IL-10 and IL-35, and negatively correlated with Th17/Treg.The levels of Th17/Treg, Th17 and IL-17 were negatively correlated with IL-35(P<0.05 to P<0.01).The results of logistic regression analysis showed that the odds ratio of severe bronchiolitis to Th17/Treg, IL-17 and IL-6 were statistically significant(P<0.05 to P<0.01).
      ConclusionsThe imbalance of Th17/Treg plays an important role in the pathogenesis of bronchiolitis.Th17/Treg, IL-7 and IL-6 are the risk factors of severe bronchiolitis.
