    硬膜外镇痛分娩方式对初产妇妊娠结局的影响

    Effect of epidural analgesia delivery on pregnancy outcome of primiparas

      目的探讨硬膜外镇痛分娩方式对初产妇妊娠结局的影响。
      方法回顾性分析436例初次分娩产妇。将其中184例采用硬膜外麻醉无痛分娩方式者纳入观察组；252例采用常规分娩方式者纳入对照组，比较2组分娩相关情况。
      结果观察组与对照组在第三产程、产后出血量、新生儿Apgar评分差异均无统计学意义（P>0.05）；观察组的第一产程时间、第二产程时间、疼痛程度、剖宫产率、住院时间、母婴并发症率均低于对照组（P＜0.05~P＜0.01）；观察组剖宫产率为5.98%，低于对照组的14.29%（P＜0.01）。
      结论硬膜外镇痛分娩方式能改善初产妇的分娩结局。

       

      Abstract:
      ObjectiveTo investigate the effects of epidural analgesia delivery on pregnancy outcome of primiparas.
      MethodsThe clinical data of 436 primiparas were analyzed retrospectively.The 184 cases treated with epidural anesthesia painless delivery and 252 cases tretaed with normal delivery were divided into the observation group and control group, espectively.The two groups were compared before and after delivery.
      ResultsThe differences of the third stage of labor, postpartum blood loss and neonatal Apgar score between two groups were not statistically significant(P>0.05).The time of the first stage of labor, time of the second stage of labor, degree of pain, rate of cesarean section, length of hospital stay and rate of maternal and infant complications in observation group were lower than those in control group(P < 0.05 to P < 0.01).The rate of cesarean section in the observation group was 5.98%, which was lower than that in control group(14.29%) (P < 0.01).
      ConclusionsEpidural analgesia can improve the delivery outcome of primipara.
