    基于Morisky量表评估的社区老年高血压病人用药依从性及影响因素研究

    Study of Morisky scale on medication compliance and influencing factors of elderly patients with hypertension in community

    • 摘要:
      目的调查社区老年高血压病人用药依从性,为社区高血压病人的健康管理提出建议。
      方法选择2020年7-9月弋矶山社区居民中60岁以上高血压病人120例,利用问卷和谈话的方式收集信息,有效调查问卷119例,1例无效。采用Morisky量表问卷的方式对病人用药依从性进行评估,分析病人用药依从性的影响因素。
      结果119例病人中52例用药依从性好,67例用药依从性差,其中因为忘记服药而导致依从性差者23例,其次是症状改善擅自停药有21例,最后是自觉症状更糟停止用药和不注意用药者23例。年龄、服药次数、居住情况、血压监测是导致病人依从性低的主要影响因素(P < 0.05)。logistic回归分析显示独居和无血压监测使病人用药依从性降低的风险增加16.9%和10.4%。
      结论医护人员和药师应加强对高血压病人的相关知识的宣传,对病人实施系统健康宣教,改善其依从性,提高高血压治疗的规范化及血压控制水平。

       

      Abstract:
      ObjectiveTo investigate the medication compliance of elderly hypertension patients in the community and put forward feasible suggestions for the health management of hypertension patients.
      MethodsA total of 120 elderly hypertension patients over 60 years old in Yijishan community from July to September 2020 were selected.Information was collected through questionnaires and interviews.One hundred and nineteen cases were effectively surveyed, but 1case was invalid.The Morisky scale questionnaire was used to evaluate patients' medication compliance, and the influencing factors of medication compliance were analyzed.
      ResultsAmong the 119 patients, 52 patients(43.7%) had good medication compliance, 67 patients(56.3%) had poor medication compliance, among which 23 cases(34.33%)had poor compliance due to forgetting to take medication, followed by 21 cases(31.34%)who stopped medication without taking medication because of symptom improvement, and 23 cases(34.33%)who stopped medication and did not pay attention to taking medication because of conscious symptom worse.Age, times of medication, residence status and blood pressure monitoring were the main influencing factors for low compliance(P < 0.05).Logistic regression analysis showed that living alone and having no blood pressure monitoring increased the risk of decreased medication compliance by 16.9% and 10.4%.
      ConclusionsMedical staffs and pharmacists should strengthen the publicity of the relevant knowledge of hypertension patients, carry out systematic health education for patients, improve their compliance, and improve the standardization of hypertension treatment and blood pressure control level.
