    改良翼状胬肉切除联合角膜缘干细胞移植原位缝合术的疗效观察

    Efficacy observation of modified pterygium resection combined with limbal stem cell transplantation in situ suture

    • 摘要:
      目的探讨改良翼状胬肉切除联合角膜缘干细胞移植原位缝合术的疗效。
      方法选取100例原发性翼状胬肉病人, 按照随机数表法分为观察组和对照组, 各50例。对照组采用常规翼状胬肉切除联合角膜缘干细胞移植手术治疗, 观察组采用翼状胬肉切除联合角膜缘干细胞移植原位缝合手术治疗。比较2组角膜切口愈合时间、结膜切口愈合时间、术后疼痛评分、病人主观不适感及复发率。
      结果手术当天及术后第1、2、3天, 2组疼痛评分均逐渐降低(P < 0.05), 且观察组各时点疼痛评分均明显低于对照组(P < 0.01)。观察组角膜切口愈合时间、结膜切口愈合时间均明显短于对照组(P < 0.01)。2组术前泪膜破裂时间差异无统计学意义(P>0.05), 观察组术后1个月泪膜破裂时间明显长于对照组(P < 0.01)。2组病人术后1个月眼部不适发生率及术后6个月复发率差异均无统计学意义(P>0.05)。
      结论翼状胬肉切除联合角膜缘干细胞移植原位缝合术中覆盖结膜瓣供区可促进翼状胬肉病人术后角膜上皮损伤恢复, 促进结膜切口愈合, 增加术后舒适度, 并能延长病人泪膜持续时间。

       

      Abstract:
      ObjectiveTo investigate the efficacy of modified pterygium resection combined with limbal stem cell transplantation in situ suture.
      MethodsA total of 100 patients with primary pterygium were selected and divide into observation group and control group according to random number table method, with 50 cases in each group.The control group was treated with conventional pterygium resection combined with limbal stem cell transplantation, and the observation group was treated with pterygium resection combined with limbal stem cell transplantation in situ suture.The corneal wound healing time, conjunctival incision healing time, postoperative pain score, patient's subjective discomfort and recurrence rate were compared between the two groups.
      ResultsThe pain scores in the two groups gradually decreased on the day of operation, on the first day, second day, and third day after operation(P < 0.05), and the pain scores in observation group at the above time-points were significantly lower than those in control group(P < 0.01).The corneal wound healing time and conjunctival incision healing time in observation group were significantly shorter than those in control group(P < 0.01).There was no significant difference in tear film rupture time between the two groups before operation(P>0.05), but the tear film rupture time in observation group was significantly shorter than that in control group at one month after operation(P < 0.01).There was no significant difference in the incidence of ocular discomfort at one month after the operation and the recurrence rate at six months after the operation between the two groups(P>0.05).
      ConclusionsPterygium resection combined with limbal stem cell transplantation in situ suture covering the conjunctival flap donor area can promote corneal epithelial injury recovery, promote conjunctival incision healing, increase postoperative comfort, and increase the tear film duration in pterygium patients.
