    CO2点阵激光预处理联合光动力疗法治疗日光性角化病临床疗效观察

    Clinical effects of CO2 fractional laser preconditioning combined with photodynamic therapy in the treatment of actinic keratosis

      目的评价CO2点阵激光预处理联合光动力治疗日光性角化病的临床疗效及其安全性。
      方法选取日光性角化病人共30例,随机分为对照组和观察组,各15例。对照组采用光动力治疗,观察组经CO2点阵激光对皮损处预处理后再行光动力治疗。2组均每1~2周治疗1次,如1次治疗后皮损未完全消退,可重复治疗,1个疗程总治疗次数不超过6次。分别于治疗结束后4周、6个月对2组病人进行随访。
      结果治疗后4周,观察组病人皮疹治疗有效率为100.00%(15/15),高于对照组的60.00%(9/15)(P < 0.05)。治疗后6个月,观察组复发率为6.67%(1/15),低于对照组的46.67%(7/15)(P < 0.05)。治疗过程中2组病人疼痛评分差异无统计学意义(P>0.05);治疗及随访过程中,2组病人均未出现严重不良事件。
      结论CO2点阵激光预处理有助于更好地清除日光性角化皮疹,有效降低皮疹复发,同时无明显不良反应。

       

      Abstract:
      ObjectiveTo evaluate the clinical efficacy and safety of CO2 fractional laser preconditioning combined with photodynamic therapy in the treatment of actinic keratosis.
      MethodsA total of 30 patients with actinic keratosis were randomly divided into the control group and observation group (15 cases in each group).The control group was treated with photodynamic therapy, while the observation group was pretreated with CO2 fractional laser, then treated with photodynamic therapy.Two groups were treated once every 1 to 2 weeks.If the skin lesions did not completely subside after one treatment, the treatment could be repeated, and the total treatment times of one course of treatment should not exceed 6 times.Two groups were followed up after 4 weeks and 6 months of treatment.
      ResultsAfter 4 weeks of treatment, the effective rate of the observation group(100.00%) was higher than that of control group(60.00%) (P < 0.05).After 6 months of treatment, the recurrence rate of the control group(46.67%) was higher than that of the observation group(6.67%) (P < 0.05).There was no statistical significance in the pain score during treatment between two groups (P>0.05), and no serious adverse events occurred in two groups during treatment and follow-up.
      ConclusionsCO2 fractional laser pretreatment can better remove the solar keratosis skin rash, effectively reduce the recurrence of skin rash, and is safe without obvious adverse reactions.
