    Effect of postoperative spiritual health on rehabilitation exercise adherence in lung cancer patients: a chain mediating effect of benefit finding and purposeful ruminative contemplation

      目的了解肺癌病人术后康复运动依从性现状,分析益处发现与目的性反刍性沉思在肺癌病人术后灵性健康对康复运动依从性影响的链式中介效应。
      方法选取203例肺癌病人为研究对象,采用一般资料调查表、疾病获益感量表、反刍性沉思问卷、慢性病治疗功能评估-灵性量表-12和康复锻炼依从性量表对其进行问卷调查;分析益处发现、目的性反刍性沉思、灵性健康与康复运动依从性之间的相关关系;采用Amos软件建立结构方程模型并进行中介效应检验。
      结果203例肺癌病人术后康复运动依从性评分(33.54±5.28)分、灵性健康评分(28.54±4.62)分、益处发现评分(62.34±8.54)分、目的性反刍性沉思评分(18.64±2.34)分。年龄、文化程度、医疗负担和居住地是肺癌病人术后康复运动依从性影响因素(P<0.01)。肺癌病人术后康复运动依从性与灵性健康、益处发现、目的性反刍性沉思之间均呈正相关关系(P<0.01)。肺癌病人术后灵性健康对康复运动依从性的链式中介效应成立,总间接效应占总效应的39.93%,其中益处发现的特定中介效应占19.91%,目的性反刍性沉思的特定中介效应占14.46%,益处发现-目的性反刍性沉思的链式中介效应占5.56%。
      结论益处发现与目的性反刍性沉思在灵性健康与康复运动依从性间的链式中介效应成立,证实灵性健康可以通过调节益处发现与目的性反刍性沉思,间接影响肺癌病人术后康复运动依从性。

       

      Abstract:
      ObjectiveTo understand the current status of postoperative rehabilitation exercise adherence in lung cancer patients and to analyze the chain mediating effect of benefit finding and purposeful ruminative contemplation on rehabilitation exercise adherence in postoperative spiritual health of lung cancer patients.
      MethodsA total of 203 postoperative lung cancer patients were selected for the study, and questionnaires were administered to them using the general information questionnaire, the revised Chinese version of benefit finding scale, the Chinese version of event related rumination inventory, the functional assessment of chronic illness therapy-spiritual and the questionnaire of exercise adherence; Pearson analysis was used to analyze the correlations between benefit finding, purposeful ruminative contemplation, spiritual health and rehabilitation exercise adherence; Amos software was used to develop structural equation models and to test the mediating effects.
      ResultsPostoperative rehabilitation exercise adherence scores, spiritual health scores, benefit finding scores, and purposeful ruminative contemplation scores in 203 lung cancer patients were (33.54±5.28) points, (28.54±4.62) points, (62.34±8.54) points, (18.64±2.34) points, respectively.Age, education level, medical burden, and place of residence were influencing factors for postoperative rehabilitation exercise adherence in lung cancer patients (P<0.01);postoperative rehabilitation exercise adherence in patients with lung cancer was positively correlated with spiritual health, benefit finding, and purposeful rumination (P<0.01).A chain mediating effect of postoperative spiritual health on rehabilitation exercise adherence in lung cancer patients was established, with the total indirect effect accounting for 39.93 % of the total effect, among which the specific mediating effect of benefit finding accounted for 19.91 %, the specific mediating effect of purposeful ruminative contemplation accounted for 14.46%, and the chain mediating effect of benefit finding-purposeful ruminative contemplation accounted for 5.56%.
      ConclusionsThe chain mediating effect of benefit finding and purposeful ruminative contemplation between spiritual health and rehabilitation exercise adherence is established, confirming that spiritual health can indirectly influence postoperative rehabilitation exercise adherence in lung cancer patients by moderating benefit finding and purposeful ruminative contemplation.
