手术治疗高颈段椎管内肿瘤18例

Surgical treatment of intraspinal tumor in the upper segment of cervial in 18 cases

摘要: 目的：探讨高颈段椎管内肿瘤手术治疗特点。方法：根据X线、CT及MRI等检查结果，初步评价肿瘤的性质、受累范围，确定手术方案。枕下颈后正中线入路15例，颈外侧入路3例，颈前方入路1例。结果：痊愈16例，1例术后无变化，1例术后呼吸困难加重，并发吸入性肺炎，术后3 d呼吸衰竭死亡。15例随访1～2年，肿瘤无复发。结论：合理的手术入路、熟练仔细的操作是手术成功的关键，显微手术是脊髓内肿瘤最有效的治疗方法。

Abstract: Objective:To discuss the characteristics of surgical treatment of intraspinal tumor in the upper segment of cervical.Methods:The surgical planning of patient was made after the character,range of tumor were evaluated according to the results of X-ray,CT and MRI.Results:After the surgical treatment,16 patients recovered,one patient had no improvement,one patient got increasing dyspnea complicated by aspiration pneumonia and died from breath failure.There was no tumor recurrence case after 1 to 2 years of follow-upin 18 patients.Conclusions:Optimal surgical approach and skillful operation are key to the treatment of intraspinal tumor,and microsurgery is the most effective method for treatment of intraspinal tumor at present.
