螺旋CT诊断外伤性肾上腺血肿的价值

Diagnosis value of spiral CT for traumatic adrenal hematomas

摘要: 目的：探讨外伤性肾上腺血肿的螺旋CT影像学表现特征及诊断价值。方法：对14例外伤性肾上腺血肿的螺旋CT影像特征进行回顾性分析。结果：14例患者中螺旋CT检查发现15个肾上腺血肿，其中右侧13例，双侧1例；局限型4例，弥漫型10例；急性期呈类圆形、椭圆形高密度，随时间推移肾上腺血肿密度逐渐减低、血肿缩小吸收；增强扫描呈无强化或边缘条弧形强化。结论：肾上腺血肿有特征性CT影像学表现，螺旋CT对肾上腺血肿诊断价值高，CT动态复查及增强扫描有助于肾上腺血肿定性鉴别诊断。

Abstract: Objective:To explore the features and value of spiral CT in diagnosis of traumatic adrenal hematomas. Methods:The spiral CT pictures of 14 cases of adrenal hematomas were analyzed retrospectively. Results:Fifteen adrenal hematomas were found in 14 the cases,13 of which were on the right side and 1 was on both sides;4 cases were localized and 10 cases diffused.At the acute stage,the adrenal hematomas were round or oval with high-density;with time going,the adrenal hematomas became smaller and absorbed;enhanced scanning showed no strengthening or arced strengthening on the edge. Conclusions:Adrenal hematomas present typical CT manifestations,and spiral CT has great value for the diagnosis.Dynamic review and enhanced scanning may be of help to the differential diagnosis of adrenal hematomas
