    Neuropsychological characteristics of subjective memory impairment and the change of cognitive event related potential P300

    • 摘要: 目的:探讨主观记忆损害(subjective memory impairment,SMI)作为遗忘型轻度认知损害(amnesia mild cognitive impairment,aMCI)前驱阶段的可能性,及事件相关电位P300对于阿尔茨海默病早期诊断和评估的临床价值。方法:应用多维度神经心理学测试对SMI组、aMCI组和正常对照(对照组)各20名受试者进行认知功能评估,对3组受试者进行事件相关诱发电位检测,比较3组受试者P300的变化特征及组间差异。结果:SMI组的AVMT-延时回忆,AVMT-即刻记忆和CDT得分均高于aMCI组和对照组(P0.05~P0.01),而aMCI组与对照组差异均无统计学意义(P0.05);SMI组和aMCI组的VFT水平明显高于对照组(P0.01),SMI组和aMCI组VFT差异无统计学意义(P0.05)。3组P300各参数间差异均无统计学意义(P0.05)。结论:神经心理学监测对于aMCI的前驱阶段SMI及aMCI早期诊断具有临床意义;P300监测对于SMI、aMCI临床诊断没有统计学意义。

       

      Abstract: ObjectiveTo investigate the subjective memory impairment ( SMI ) in prodromal stage of amnestic mild cognitive impairment( aMCI) and the clinical value of P300 of event-related potentials in early diagnosis and evaluation of Alzheimer's disease.Methods: The cognitive function of the SMI group,aMCI group and control group ( 20 cases each group ) were assessed using multidimensional neuropsychological testing, the event-related potentials of three groups were detected.The change characteristics and differences of P300 between three groups were analysed.Results: The AVMT-delay memory recall,AVMT - immediate memory and CDT in SMI group scores were higher than the aMCI group and the control group( P 0.05 - P 0.01) ,but there was no significant difference between the aMCI group and the control group( P 0.05) ; the level of VFT in SMI and aMCI group were significantly higher than the control group ( P 0.01) , the VFT in SMI group and the aMCI group showed no statistical difference ( P 0.05) .The connection testing,DS and CFT copying score test in the three groups had no statistical difference ( P 0.05) . Conclusions: The neurophysiological testing has important clinical value in early diagnosis of aMCI and SMI in prodromal stage,but P300 testing has no value for SMI and aMCI clinical diagnosis.
