    Application of plasma cystatin C in assessing glomerular filtration rate

    • 摘要: 目的: 探讨胱蛋白酶抑制剂C(cystatin C)在评估肾小球滤过率中的价值。方法: 将109例患者按内生肌酐清除率(Ccr)水平分4组:肾功能正常组(Ccr80 ml/min)33例,肾功能不全代偿组(50 ml/minCcr80 ml/min)20例,肾功能失代偿组(20 ml/minCcr50 ml/min)27例,肾功能衰竭组(Ccr20 ml/min)29例;另设正常对照组69名。分别检测观察对象血浆cystatin C、血肌酐(Scr)、尿素氮水平,计算其Ccr,并将结果进行统计学分析。结果: 其他肾功能组cystatin C结果与健康对照组比较,差异均有统计学意义(P<0.01),并随着Ccr的下降而上升,且与Ccr和Scr均具有较好的相关关系(P<0.01)。结论: 血浆cystatin C检测是判断肾小球滤过功能的敏感指标,其结果可替代内生Ccr。

       

      Abstract: Objective: To explore the value of cystatin C in assessing glomerular filtration rate.Methods: One hundred and nine patients were devided into 4 groups according to the level of endogenous creatinine clearance rate(Ccr),which included normal renal function group(Ccr80 ml /min,n = 33), renal insufficiency compensation group (50 ml /minCcr<80 ml /min,n = 20), renal insufficiency decompensation group(20 ml /minCcr<50 ml /min,n = 27)and renal failure group(Ccr<20 ml /min,n = 29).Sixtynine healthy people were set as control group.The levels of cystatin C,Scr and urea nitrogen in blood plasma of all group were determined,which were statistically analyzed.Results: Compared with control group, there were significant differences in the cystatin C levels of other groups (P<0.01).The levels of Cystatin C increased along with the Ccr levels decline.There were good relevant relationships between Ccr and Scr (P<0.01).Conclusions: Blood plasma cystatin C is a sensitive indicator for evaluating the glomerular filtration function,which can replace the role of Ccr.
