    Expression of vascular endothelial growth factor in vascularized bone transplanationreconstituted bone xeno graft transfer for repair of bone defection

    • 摘要: 目的: 探讨血管化骨重组异种骨(RBX)联合移植修复骨缺损的疗效及受体血清血管内皮生长因子(VEGF)的表达状况,作为移植诱骨效应的评价参考指标。方法: 27例骨缺损随机分为血管化骨RBX移植修复组(A组,9例);血管化骨移植修复组(B组,10例);RBX移植修复组(C组,8例)。临床评价以术后3、6、12个月随访的X线片来判断骨缺损是否修复成功、骨愈合时间长短、是否再吸收等。3组分别在术前及术后2周、4周、6周、8周采用发光免疫分析法测定受体血清VEGF的表达。结果: 术后3个月A组8例、B组6例、C组3例骨愈合,6个月A组1例、B组3例、C组3例延迟骨愈合,12个月B组1例、C组2例发生骨移植区部分再吸收,3组临床疗效差异无显著性(P>0.05)。3组术后第2、4周血清VEGF值均较术前升高(P<0.01),且A、B组均高于C组(P<0.01)。术后第6、8周3组的血清VEGF值术前后及各组间比较差异均无显著性(P>0.05)。结论: 血管化骨RBX组的骨折愈合率明显优于单纯血管化骨移植和RBX移植。血管化骨RBX组和血管化骨组的血清VEGF的值均明显高于术前及单纯RBX移植组。移植早期受体血清VEGF表达明显增高,其对移植诱骨效应的评价具有临床参考价值。

       

      Abstract: Objective: To study the efficiency of vascularized bone graft combined with reconstituted bone xeno graft(RBX) in repairing the bone defection and the expression of the vascular endothelial growth factor(VEGF) as an evaluation index of bone graft success.Methods: Twenty-seven cases of bone defection were randomly divided into vascularized bone graft-RBX group,vascularized bone graft group and RBX group.The clinical results were evaluated by the radiograph 3 months,6 months and 12 months after operation according to whether the bone defection was repaired,how long it took the bone to heal and whether the bone graft died away or not.The expression of VEGF in serum was assayed with immuno analysis method in the 3 groups 2 weeks,4 weeks,6 weeks and 8 weeks before and after the operation.Results: The bone defections were repaired in 8 cases,6 cases and 3 cases in the vascularized bone graft-RBX group,vascularized bone graft group and RBX group,respectively,3 months after the operation.One case,3 cases and 3 cases of bone defections in the vascularized bone graft-RBX group,vascularized bone graft group and RBX group achieved delayed recovery respectively 6 months after the operation.One case in the vascularized bone graft group and 2 cases in the RBX group developed part absorption in the bone transplantation area 12 months after the operation.The difference was not significant(P>0.05).Serum VEGF value was higher 2 weeks and 4 weeks after the operation than before the operation in all the 3 groups,which was more obvious with the vascularized bone graft-RBX group and vascularized bone graft group (P<0.01).The serum VEGF value 6 weeks and 8 weeks after the operation had no significant difference between the groups(P>0.05).Conclusions: Vascularized bone graft-RBX is more effective compared to the other 2 methods in repairing bone defections.After the operation,serum VEGF value in the vaschlarized bone graft-RBX group and the vascularized bone graft group increased,compared with that of the RBX group.The expression of serum VEGF increased obviously in the earlier period of bone transplantation.It is of value for clinical evaluation of the efficiency of bone defection repair with bone graft.
