胃癌术后化疗病人癌因性疲乏影响因素分析及护理对策

    Analysis of influencing factors and nursing strategies of cancer-induced fatigue in patients with gastric cancer after chemotherapy

      目的 分析胃癌术后化疗病人癌因性疲乏的影响因素及护理对策。
      方法 选择121例胃癌术后化疗病人作为研究对象,结合病人人口学资料,通过领悟社会支持量表、痛苦表露量表、心理弹性量表探讨胃癌术后化疗病人癌因性疲乏影响因素,以便提出护理对策。
      结果 胃癌术后化疗病人癌因性疲乏分数为(39.46±9.35)分,其中年龄、教育程度、月收入与胃癌术后化疗病人癌因性疲乏的发生有关(P < 0.01),经Pearson检验显示癌因性疲乏与领悟社会支持(r=-0.545,P < 0.01)、痛苦表露(r=-0.418,P < 0.01)、心理弹性(r=-0.564,P < 0.01)均呈明显负相关关系。多重线性回归分析显示,年龄教育程度、月收入、领悟社会支持、痛苦表露及心理弹性是此类病人癌因性疲乏发生的独立影响因素(P < 0.05~P < 0.01)。
      结论 老年、教育程度低、低收入的胃癌术后化疗病人易发生癌因性疲乏,护士可因人而异开展针对性护理措施,如开展叙事医学技术,鼓励病人配偶、子女或朋友分担责任,给予社会支持;运用同理心、移情、启发等方式联合同伴支持教育,提高病人自我表露水平;学习正念减压技术来降低此类病人癌因性疲乏的发生。

       

      Abstract:
      Objective To analyze the influencing factors and nursing strategies of cancer-induced fatigue in patients with gastric cancer after chemotherapy.
      Methods A total of 121 gastric cancer patients with postoperative chemotherapy were selected as study subjects. Combined with the demographic data of patients, the influencing factors of cancer-related fatigue in patients with postoperative chemotherapy for gastric cancer were investigated by social support scale, pain expression scale and mental resilience scale, so as to put forward nursing strategies.
      Results The score of cancer-induced fatigue in patients with gastric cancer after chemotherapy was (39.46±9.35) points, and the age, education level and monthly income were related to the occurrence of cancer-induced fatigue in patients with chemotherapy after gastric cancer after chemotherapy (P < 0.05). The results of Pearson′s test showed that the cancer-related fatigue was negatively correlated with comprehension of social support(r=-0.545, P < 0.01), pain expression(r=-0.418, P < 0.01) and psychological resilience(r=-0.564, P < 0.01). The results of multiple linear regression analysis showed that the age, education level, monthly income, comprehension of social support, pain expression and psychological resilience were the independent influencing factors of cancer-induced fatigue(P < 0.05 to P < 0.01).
      Conclusions Elderly, low-education and low-income patients with gastric cancer after chemotherapy are prone to the cancer-induced fatigue. Nurses can carry out targeted nursing measures for different patients, such as developing narrative medicine techniques, encouraging patients′ spouses, children or friends to share responsibilities and provide social support. Using empathy, inspiration and other ways to cooperate with peer support education to improve patients′ self-disclosure level and learn mindfulness-based stress reduction techniques can reduce the incidence of cancer-induced fatigue in these patients.
