Effect of the hospital-community linkage nursing management mode on the rehabilitation and quality of life in patients with stroke

Objective:To explore the effects of the hospital-community linkage nursing management mode on the rehabilitation and quality of life in patients with stroke.Methods:One hundred and twenty stroke patients were randomly divided into the observation group and control group(60 cases each group).The control group was taught with the routine health mode,and the observation group were taught with hospital-community linkage nursing management mode based on the control group for 6 months.The functional status,activities of daily living,neurological deficit before intervention,after 3 and 6 months of intervention,and the quality of life before intervention and after 6 months of intervention between two groups were compared.Results:After 3 and 6 months of intervention,the scores of functional status and activities of daily living in observation group were significantly higher than those in control group(P<0.01),while the score of NIHSS in observation group was significantly lower than that in control group(P<0.01).The scores of functional status and activities of daily living in two groups after intervention were significantly higher than those before intervention,and the score of NIHSS in two groups after intervention was significantly lower than that before intervention(P<0.05 to P<0.01).After 6 months of intervention,the total scores of physical life,physical function,mental function,social function and social quality in observation group were significantly higher than those in control group(P<0.01).Conclusions:The hospital-community linkage nursing management mode can effective promote the recovery of limb function,reduce neurological deficits,and improve the quality of life of patients with stroke.
