Effect of self-care theory combined with collaborative care model on health behavior and self-care agency of discharged patients with cerebral infarction

    • ObjectiveTo explore the effect of self-care theory combined with collaborative care model on the health behavior and self-care agency of discharged patients with cerebral infarction (CI).
MethodsA total of 200 discharged CI patients from October 2016 to September 2018 were selected, and divided into observation group and control group according to the time of discharge(100 cases in each group). The control group was treated with routine nursing, and the observation group was treated self-care theory combined with collaborative care model on the basis of control group for 6 months. The treatment compliance was recorded, and the health behavior, self-care agency and nursing satisfaction were compared before intervention and after 6 months of intervention in two groups.
ResultsThe compliance rate of diet control, exercise and drug treatment in observation group was higher than that in control group(P < 0.05).After 6 months of intervention, the scores of stress response, exercise, health responsibility and self-achievement in observation group were higher than those in control group (P < 0.01);the scores of self-responsibility, self-concept, health knowledge level and self-care skill were higher than those in control group (P < 0.01);the scores of activity of daily living, mobility, memory and thinking, and emotion in observation group were higher than those in control group (P < 0.01);and the satisfaction of nursing work in observation group was higher than that in control group (P < 0.05).
ConclusionsThe application of self-care theory combined with collaborative care model in discharged CI patients is helpful to improve treatment compliance, establish health behavior, enhance self-care agency, improve quality of life, and increase satisfaction of nursing work.
