    Application of failure mode and effect analysis combined with root cause analysis in the management of ICU patients with fecal incontinence-related dermatitis

    • ObjectiveThe failure mode and effect analysis combined with root cause analysis were used to improve the management and measures of ICU patients with fecal incontinence-related dermatitis, and the effect of its application was tested.
      MethodsUsing the convenience sampling method, 160 patients with fecal incontinence in the ICU ward of a tertiary hospital in Anhui Province were selected as the research objects, 81 patients from January to June 2020 were selected as the control group, and 79 patients from July to December 2020 were selected as the experimental group.The control group adopted the routine nursing mode, while in the observation group, failure mode and effect analysis method was implemented, incontinence management group was established, flow chart was drawn, failure modes were listed and graded, root cause analysis was used to find the root cause of failure modes, and measures were taken to improve the problems that need to be solved first..The incidence, severity and RPN value of IAD before and after intervention were compared between the two groups.
      ResultsThe incidence of IAD in the control group was 39.5 %, including 14 cases of grade 1 IAD and 18 cases of grade 2 IAD; The incidence of IAD in the observation group was 24.1%, including 3 cases of grade 1 and 16 cases of grade 2 IAD; The incidence and severity of IAD in the observation group were lower than those in the control group (P < 0.05).The risk priority index of the observation group after intervention was 10 items lower than that of the control group, with a statistically significant difference (P < 0.05).
      ConclusionsThe application of failure mode and effect analysis combined with root cause analysis in the management of fecal incontinence patients can accurately analyze the potential failure mode and risk causes, formulate targeted management and improvement measures, and effectively reduce the occurrence of IAD.
