ObjectiveTo investigate the effects of chronic disease trajectory framework intervention program on the health literacy, self-care ability and quality of life in patients with heart failure.
MethodsSixty-seven patients with chronic heart failure were randomly divided into the observation group(34 cases) and control group(33 cases).The control group was treated with the routine nursing intervention, while the observation group were treated with the chronic disease trajectory framework intervention program.The health literacy, self-care ability and quality of life between two groups before and after the intervention were evaluated using the heart failure specific health literacy scale(HFS-HLS), self-care of heart failure index(SCHFI) and Minnesota living with heart failure questionnaire(MLHFQ).
ResultsBefore intervention, there was no statistical significance in the scores and total scores of HFS-HLS, SCHFI and MLHFQ between two groups(P > 0.05).After 3 months of intervention, the scores of HFS-HLS, SCHFI and MLHFQ in the observation group were significantly improved compared with those before intervention, and the scores and total scores of HFS-HLS and SCHFI in the observation group were significantly higher than those in control group(P < 0.01), while the scores and total scores of MLHFQ in the observation group were significantly lower than those in control group(P < 0.01).
ConclusionsThe effects of chronic disease trajectory framework intervention program in chronic heart failure patients is better than that of conventional nursing, which is helpful to improve patients' self-care ability and health literacy level, and improve the quality of life.
