    Study on the relationship between the rs3758391 mutation in the promoter region of SIRT1 gene and susceptibility and efficacy of different grades of elderly hypertension

    • ObjectiveTo investigate the relationship between the mutation of rs3758391 in the promoter region of the silent information regulator 1(SIRT1) gene and susceptibility of different grades of elderly hypertension, and relationship between genotype and therapeutic effects of nifedipine.
      MethodsTwo hundred and twenty elderly hypertensive patients and 185 healthy subjects were divided into the observation group and control group, respectively.The rs3758391 polymorphism of SIRT1 gene was detected using the TaqMan method, and the relationship between SIRT1 gene polymorphism and senile essential hypertension was analyzed.The observation group was treated with nifedipine.The levels of SBP and DBP in patients with different genotypes were compared before and after treatment, and the predictive value of genotypes for curative effect was analyzed.
      ResultsThere were two alleles of T and C at the rs3758391 locus of the SIRT1 gene.The differences of the genotype and allele frequency distribution between two groups were statistically significant(P < 0.01), and the differences of the genotype and allele frequency distribution in different hypertension grades patients of observaion group were statistically significant(P < 0.01).After nifedipine treatment, the SBP and DBP of patients with grade 1-3 hypertension in the observation group significantly decreased compared with before treatment(P < 0.01), and those in TT genotype patients were lower than which in CT and CC genotype patients(P < 0.05 to P < 0.01).Among 220 patients, 34 cases were poorly controlled and 186 cass were well controlled.The good control rate of TT patients was higher than that of CT+CC patients(P < 0.05).
      ConclusionsThe rs3758391 mutation in the promoter region of SIRT1 gene is closely related to the susceptibility of the elderly hypertension in different grades and blood pressure control effect.The patients with C allele have higher hypertension grade and poor blood pressure control effect, so the SIRT1 can be considered as a marker to evaluate the efficacy of hypertension.
